UNIT 1 STEAM GENERATOR EDDY CURRENT Ul R17 Refueling Outage 1.0 Summary This report is intended to satisfy the requirements of PVNGS Technical Specifications 5.6.8 for the submittal of a Steam Generator Tube Inspection Report. The steam generator (SG) eddy current examination for the 17th refueling outage in Unit 1 (UIRI7) was conducted, during April 2013.

Mode 4 entry of Unit 1 was entered on April 24, 2013. The initial examination plan for both steam generators is listed in Table 1. This table summarizes the examinations performed for.each of the various categories, examination types, extents, and the number of tubes or tube locations completed.

This was the fourth examination performed in Unit 1 following steam generator replacement in U1R12. This examination is considered a 100% full length tubing inspection.

The examinations resulted in a total of 11 tubes being plugged in SG 11, and 27 tubes being plugged in SG 12. A description of the previous plugging history is noted in Table 2, and Appendix E provides a map of all tubes plugged. It should be noted that the plugging criteria utilized during this outage again supports 2 cycles of operation (next scheduled eddy current examinations are for R19).

2.0 Scope of Examinations Performed The original examination plan was developed based on the "PVNGS Steam Generator Degradation Assessment" developed per PVNGS Procedure 81 DP-9RC0 1 as required by NEI 97-06. In addition, possible damage mechanisms were reviewed along with the specific requirements set forth in Procedure 73TI-9RCO0 and the PVNGS Technical Specifications. The plan was finalized to include 100% bobbin examinations.

This original plan, along with the examinations performed as a result of bobbin indications noted, is summarized in Table 1 of this report.

3.0 Active Degradation Mechanisms The only degradation noted during the examinations was determined to be wear. Section 8.0 contains further discussions relating to this mechanism. Table 2 summarizes the results into categories and sections B and C itemizes all indications reported.

Westinghouse Anser software was utilized to acquire the data along with the Pegasys robotic manipulator. This robot was configured with a dual guide tube in each of the hot and cold legs.

The tubing was examined with Zetec manufactured bobbin coil probes and Zetec rotating coil (RC) style probes. Probe diameters were 0.590" to 0.610". Plus Point RC probes were used for the characterization of non-quantifiable or distorted bobbin indications.

Fiber optic cable was used from containment to the data acquisition room located at the PVNGS North Annex. Primary and secondary analysis was all performed on site. The Primary and Secondary Resolution Analysts, Independent Review Analysts, and data management were also located at PVNGS in the North Annex. Westinghouse provided the data acquisition and secondary data analysis. Anatec International, Inc. provided the primary data analysis.

Each individual from Westinghouse and Anatec International, Inc. who performed data analysis was required to complete and pass a PVNGS site specific Eddy Current Data Analysis Course as well as an associated performance and written examination. All individuals performing data analysis were also required to have Qualified Data Analyst (QDA) certification.

5.0 Indication Summary A detailed listing of the location and measured sizes of indications recorded is included in Appendix B and C. A summary of these indication results is located in Table 2. In addition, Appendix A contains a reference drawing of steam generator support locations and report legend.

There were no indications that were identified as linear during this outage.

6.0 Tubes Plugged A summary of the tubes plugged is located in Table 2. Only 11 tubes in SG 11 and 27 tubes in SG 12 were plugged this outage.

Appendix E contains a map that details the plugged tube location along with the previously plugged tubes.

7.0 Plug History A summary of the number and percentage of tubes plugged is also located in Table 2.

8.0 Condition Monitoring Tube Inspections Per the Steam Generator Program; as defined in PVNGS Procedure 81 DP-9RCO 1, a condition monitoring evaluation was conducted by PVNGS Engineering. The results of the eddy current examinations are provided in Section 5.0. An engineering evaluation of the as-found condition of inservice tubes did not reveal any degradation exceeding the threshold values for structural and leakage integrity. As such, all steam generator performance criteria were satisfied for Unit 1 Cycles 16 and 17. No tube pulls or insitu pressure testing were required based on the results of the examinations.

Foreign Obiect Search and Retrieval (FOSAR)

Tubesheet Annulus and Blowdown Lane Foreign Object Search and Retrieval (FOSAR) was conducted using a power cart mounted with a remotely operated camera and retrieval tooling. The applicable requirements of Revision 3 of the EPRI Steam Generator Integrity Assessment Guidelines Section 10.5, Secondary Side Visual Inspections, were applied for the FOSAR inspections. Sludge lancing was not performed during 1R17. Little sludge was observed in the tubesheet annulus region and in the blowdown lane of both SGs.

Also included in the scope of the FOSAR effort was an inspection of the blowdown patch plate welds that were found to be cracked in Unit 1 during U1RI 5. The condition was first identified in Unit 2 during U2R15. The inspections confirmed that the weld material in the vicinity of the cracked weld on all 4 patch plates (2 per SG) is intact, and a loose parts concern is not being created. A previous evaluation concluded that, with the presence of the cracked welds, the patch plates in the Unit 2 SGs will continue to perform their design function and that the probability of loose parts being formed is remote. This evaluation also applies to Unit 1. Thus, there is a very low 5 of 68 U1R17

risk that the cracked welds will affect the structural or leakage integrity of tubes in these steam generators.

SG 1I - Foreign obiect Summary The only foreign object observed in SG1 1 during U1R17 was what appeared to be a metal turning machining remnant. The machining remnant was identified from the 270 hand hole hot leg side of the'divider plate near the Dome area. The part was retrieved. CRDR 4390595 was initiated to document the discovery of this foreign object into the PVNGS corrective action program. No wear was associated with this foreign object.

Three tubes (R32C79, R33C80, and R32C81) were reported to contain new PLP signals just above the tubesheet in the blowdown lane hot leg side in SG 11 from the eddy current inspection program.

There were no wear indications and according to the eddy current response, the material appears to be similar to sludge. RPC examinations were performed on surrounding tubes and verified that there was no wear or other part detection in the area. This tube location is inaccessible during FOSAR inspections, so FOSAR could not be performed. PVNGS has historically taken the position that if a foreign object is detected by ECT or FOSAR without the presence of wear, it is reasonable to conclude that the required conditions.to promote wear do not exist. Per the PVNGS SG Program, trending of these locations will continue in future outages.

Three tubes (R164C175, R165C76, and R3C192) were reported to contain repeat Potential Loose Part (PLP) signals (i.e., these signals were also present in U1R15). There was no wear associated with these tubes. PVNGS has historically taken the position that if a loose part is detected by ECT or FOSAR without the presence of wear, it is reasonable to conclude that the required conditions to promote wear do not exist. Per the PVNGS SG Program, trending of these locations will continue in future outages.

SG12 - Foreign obiect Summary There were three loose parts observed in SG 12. ECT identified indications of a loose part on tubes RIC172, R2C173 and R3C172 and indications of wear on tubes R3C172 and tube R2C173 just above the hot leg top of tubesheet in the blowdown lane in SG 12. RPC testing . confirmed volumetric degradation on these tubes. Using ETSS 27091.2, the flaw on tube R3C172 measured 24% TW. The initial FOSAR performed in the BD lane did not identify a loose part in this area.

After the indications were reviewed, another FOSAR inspection was performed (using a different camera) in the area of RIC172 and the part was identified from two different angles. Wear indications could be seen by the FOSAR inspection as well. The part was retrieved and it appears to be weld splatter. Although the part was removed, the area around the foreign object (9 tubes) was plugged and staked as a conservative measure. CRDR 4390595 was initiated to document the discovery of this foreign object into the PVNGS corrective action program.

The second part was identified by ECT in the area of RIOCI on the Flow Distribution Plate (FDP).

Visual inspection of the FDP in the area of RIOCI was performed and the object was determined to be tube scale. Although tube scale is not considered to be a material that causes tube wear, the area around the tube scale was plugged (3 tubes) since the material could not be retrieved. CRDR 4385427 was initiated to document the discovery of this tube scale into the PVNGS corrective action program.

A third loose part was found in SG 12 in the cold leg annulus region between tubes R168C83 and R167C82 on top of the tubesheet. The part was retrieved and has been identified as a half of a feedwater heater castle nut. There was no damage to any of the surrounding tubes. CRDR 4390595 6 of 68 U1R17

was initiated to document the discovery of this foreign object into the PVNGS corrective action program.

Two tubes(R105C 104, R108C 105) were reported to contain repeat PLP signals (i.e., these signals were also present in UIR15). There was no wear associated with these tubes. PVNGS has historically taken the position that if a loose part is detected by ECT or FOSAR without the presence of wear, it is reasonable to conclude that the required conditions to promote wear do not exist. Per the PVNGS SG Program, trending of these locations will continue in future outages.

Upper Internals Inspection Summary Inspections were performed on both SG 11 and SG 12 Upper Internals.-The examinations provided visual documentation of the overall condition of the components in the accessible areas above the primary separator deck, below the primary separator deck (inside the shroud) and the inteftial-feed ring piping (outside the shroud). The upper hand hole (HH#5) was opened and a camera was inserted into the no-tube lane to inspect the top of the 7 th eggcrate tube support, the bottom of the 8 th eggcrate tube support and the center stay opening. No foreign objects were found during the upper internals inspection or during the inspection in'HH#5.

Specific areas examined during the inspection above the primary separator deck included physically attempting to move each of the 194 steam separators (cans), inspection of all drain lines, bottom of the dryer deck, bottom of the dryers and accessible nozzles. One loose steam separator was found in SG 11. The bolting showed no exposed threads, whereas tight steam separators' bolts showed three exposed threads. The loose steam separator was repaired under WO 4077991 and the condition was documented in PVAR 4384119.

Specific areas examined during the Upper Internals inspection below the can deck included tube bundle observation for deposits, bottom of the steam separators, examination for erosion or corrosion of the accessible discharge nozzles on the feedwater and recirculation piping. Some deposits were noted on tubes in the upper bundle region and appeared to be more concentrated near the diagonal supports. The visual inspection of the internal feedring piping and discharge nozzles did not note any erosion or corrosion. The visual inspection of the wet layup recirculation piping revealed that the piping was covered in magnetite, however no severe erosion or corrosion was noted and the piping seemed to be in acceptable condition. The bottom of most of the steam separators showed some deposits but did not show any blockages.

During the inspections via HH #5 some deposits were noted in the tube bundle, and no other conditions were noted. The tube supports were in acceptable condition based on this inspection.

Finally, the EPRI PWR Steam GeneratorExamination Guidelines require that a visual inspection of the previously installed steam generator plugs be performed to assess plug integrity. Additionally, the Examination Guidelines require a verification of the location and presence of existing in-service plugs. The conduct of the plug location and integrity verification was performed in U 1R 17 per procedure 81 CP-9RC40. A review of the inspection results indicated that all plugs were accounted for and no evidence of potential plug leakage was identified.
