ES-401 WC-2013-07 Written Examination Review Worksheet Form ES-401-9 Instructions
[Refer to Section D of ES-401 and Appendix B for additional information regarding each of the following concepts.]
- 1. Enter the level of knowledge (LOK) of each question as either (F)undamental or (H)igher cognitive level.
- 2. Enter the level of difficulty (LOD) of each question using a 1 B 5 (easy B difficult) rating scale (questions in the 2 B 4 range are acceptable).
- 3. Check the appropriate box if a psychometric flaw is identified:
The stem lacks sufficient focus to elicit the correct answer (e.g., unclear intent, more information is needed, or too much needless information).
The stem or distractors contain cues (i.e., clues, specific determiners, phrasing, length, etc).
The answer choices are a collection of unrelated true/false statements.
The distractors are not credible; single implausible distractors should be repaired, more than one is unacceptable.
One or more distractors is (are) partially correct (e.g., if the applicant can make unstated assumptions that are not contradicted by stem).
- 4. Check the appropriate box if a job content error is identified:
The question is not linked to the job requirements (i.e., the question has a valid K/A but, as written, is not operational in content).
The question requires the recall of knowledge that is too specific for the closed reference test mode (i.e., it is not required to be known from memory).
The question contains data with an unrealistic level of accuracy or inconsistent units (e.g., panel meter in percent with question in gallons).
The question requires reverse logic or application compared to the job requirements.
- 5. Check questions that are sampled for conformance with the approved K/A and those that are designated SRO-only (K/A and license level mismatches are unacceptable).
- 6. Enter question source: (B)ank, (M)odified, or (N)ew. Check that (M)odified questions meet criteria of ES-401 Section D.2.f.
- 7. Based on the reviewer=s judgment, is the question as written (U)nsatisfactory (requiring repair or replacement), in need of (E)ditorial enhancement, or (S)atisfactory?
- 8. At a minimum, explain any AU@ ratings (e.g., how the Appendix B psychometric attributes are not being met).

We dont think that the BIT is credible in this question for distractor C. This appears to be a new question (not a modified bank) since you are not asking the same information as the original question.

S Reference does not completely support the answer as provided. The discussion of the correct answer (ie why it is correct) in the pedigree portion of the worksheet is also lacking. There would be very little difference between temperature of letdown stream entering TV-130 and CCW temp. I believe VCT temp may not change. VCT pressure probably controlled by H2 PCV.

D doesnt seem credible to me. If the valve fails as is when it reaches its full open position, this could be argued correct on appeal. Also, since nothing is given in the stem to indicate another failure that would cause it to fail as is, this is not credible.

KA mismatch-This question is really asking do you know that auto swap-over occurred at 8% RWST level, not the design flow rates of the RHR pumps. I would recommend something in the stem that could actually cause the pump to approach run-out conditions and give that flow rate and see if they recognize that they have exceeded the design limit or if they dont close a valve or take some action then they would exceed the design limit for flow rate.
How does flow change before vs after cold leg swap-over?
WC investigate RHR design and possible KA replacement. Question is still unsat.
Distracters C and D are not plausible. (running at shutoff head) Question changed.

S Typo in worksheet, system is PRTS, not PS.
Why would you use ALR-00-034D to reduce pressure when this procedure directs you to other procedure/step in effect if PRT temp is not greater than 120 degrees (it is 100 deg if first part of answer A is correct)? Is it plausible to add water to cool a tank that is at 100 degrees? D is not credible for this reason. These also looks like a GFES question in disguise to me. Vent excess pressure to different tanks could be one way to fix this question. Another might me to see if they know when the PRT bellows fails (ie what pressure).
I would expect temp and pressure to both be elevated from normal. A answer does not seem reasonable. I am having trouble following the logic for the distracters.

S You cant add a strainer to a system that does not exist to create a distracter (such as C). Also, keep the structure the same for all four selections. For B state Anti-vortex devices in the sump suction create a centrifugal forceAlso try to keep the length the same when doing this. Another way to write this question is to ask it in a way to know the size of the nozzle holes (if that is required knowledge) and the debris size expected to make it through whatever choices you give them.
B needs to be reworded. Anti vortex prevents the swirl, keeping air out of the center.

S C 43.5, 2 A and D are not plausible. Which TS really is more restrictive. Leak rate must be pretty large for letdown to have to be isolated. What causes alarm 35A? Is the Safety Valve really inoperable?

D 43.5 A is also correct with stem as written. Stem does not differentiate whether or not OFN BG-0009 should be used. It only asks for the difference between the two procedures. RO knowledge.
K/A mismatch. Question is asking how procedures differ, not how they are used together. Not reasonable for applicants to think PORVs are opened.

K/A mismatch. This question does not deal with procedures for inadvertent SI. SI pump flow path is simple RO knowledge.
Upon further review, this is a mode 3 question with a complicated lineup that is only SRO knowledge per licensee. Accepted as is.
