Dear Sir or Madam:

This is notification that Crane Nuclear, Inc. has been advised by our customer about leakage on'the body of an installed 2" valve assembly. The 2" valve body identified as leaking or weeping was cast from alloy SB 148 UNS 95200. Crane Nuclear has not corroborated the potential leakage and our customer has returned two valve assemblies for our evaluation.

To date the potential leaking has been isolated to a body on a 2" valve assembly. Crane Nuclear has determined through investigation that this unique alloy cast material has been provided to only one customer, Dominion /

Millstone, over two purchase orders as noted below.

40224-04 E1630, E1631, E1632, E1633, E1634, E1635, E1636 Each valve assembly above was subjected to a hydrostatic shell test performed in accordance with the ASME B&PV Code requirements, and witnessed by our Quality Control personnel. There was no weeping or valve body leakage noted on any of the valves during the duration of the hydrostatic shell or seat leakage test.

The potential failure mode is pressure boundary violation and loss of inventory however; Crane Nuclear is not able to quantify the loss of inventory or subsequent safety issues without application specifics.

While it was the customer that identified the valve body leakage to Crane Nuclear, a notification letter with the information above was sent to Dominion / Millstone today in accordance with their purchase order notification requirements.
