U Area Walk-By Checklist (AWC)

Location (Bldg, Elev, Room/Area):

AWC-U3-14 Instructions for Completing Checklist This checklist may be used to document the results of the Area Walk-By near one or more SWEL items. The space below each of the following questions may be used to record the results of judgments and findings.

Additional space is provided at the end of this checklist for documenting other comments.

1. Does anchorage of equipment in the area appear to be free of potentially Yes adverse seismic conditions (if visible without necessarily opening cabinets)?
2.

Does anchorage of equipment in the area appear to be free of significant Yes degraded conditions?

Minor corrosion in a number of anchorages judged to be acceptable.

3.

Based on a visual inspection from the floor, do the cable/conduit raceways and Yes HVAC ducting appear to be free of potentially adverse seismic conditions (e.g.,

condition of supports is adequate and fill conditions of cable trays appears to be inside acceptable limits)?

4.

Does it appear that the area is free of potentially adverse seismic spatial Yes interactions with other equipment in the area (e.g., ceiling tiles and lighting)?

5.

Does it appear that the area is free of potentially adverse seismic interactions Yes that could cause flooding or spray in the area?

Threaded and welded fire piping is well-supported and judged to be acceptable. Threaded unit heater piping is away from equipment and near drainage trough and judged to be acceptable.

6.

Does it appear that the area is free of potentially adverse seismic interactions Yes that could cause a fire in the area?

Flammable items stored in locked flammable material storage cabinet.

7.

Does it appear that the area is free of potentially adverse seismic interactions Yes associated with housekeeping practices, storage of portable equipment, and temporary installations (e.g., scaffolding, lead shielding)?

Tall scaffold is restrained against tipping and sliding. Emergency eye wash station is greater than 24 inches from SW piping valves in non-tip direction. A robust, non-Category 1 support lies in the tip direction. All are judged to be acceptable.

1. Does anchorage of equipment in the area appear to be free of potentially Yes adverse seismic conditions (if visible without necessarily opening cabinets)?
2.

Does anchorage of equipment in the area appear to be free of significant Yes degraded conditions?

Minor floor coating failures judged to be acceptable.

Minor gaps between base plate and wall due to uneven concrete judged to be acceptable.

3.

Based on a visual inspection from the floor, do the cable/conduit raceways and Yes HVAC ducting appear to be free of potentially adverse seismic conditions (e.g.,

4.

Does it appear that the area is free of potentially adverse seismic spatial Yes interactions with other equipment in the area (e.g., ceiling tiles and lighting)?

S-hooks and/or eyehooks in light fixtures need to be more adequately closed.

Fixtures might swing down from their non-power cord ends but will not affect soft targets, and therefore are judged to be acceptable.

IR 1396017:

LIGHT SUPPORT CONNECTION DEFICIENCY During the Seismic Walkdowns it was identified that there are multiple open S-hook connections between the chain and the support bracket for the lighting fixtures in the U3 HPCI Room. However, each light fixture is hardwired, therefore, they would not fall if the support chains were to hardwired, therefore, they would not fall if the support chains were to come loose. In addition, the equipment within the range that the light fixtures would swing, if they were to come loose, appears to be robust and would not be significantly damaged by engineering judgement.

5.

Does it appear that the area is free of potentially adverse seismic interactions Yes that could cause flooding or spray in the area?

Threaded fire piping well supported and judged to be acceptable. Welded piping judged to be acceptable.

Collection pit below grating at turbine near end of room is full of fluid which appears to be water. Fluid in pit does not pose a flood or spray risk.

Permanent scaffold and trolley chain fall are properly restrained.

Sump pump equipment is adequately stored away from plant equipment.
