SRO-Tier 2/Group 1 lO3/02.04.08 Duplicate generic RO KIA, no containment AOP, reselected 2.4.6 (JST S/30/12)
SRO-Tier l/Group 1 038.EA2.0S Shrink and Swell subject similar to RO 60. Randomly selected.07 (JST 617/12)
SRO-Tier lIGroup 2 068.AA2.02 AOP-1011 OA doesn't have operators use local boric acid flow. Randomly selected.03 (JST 617112)
SRO-Tier lIGroup 1 054/02.04.04 Entry level conditions for EOP and AOP is RO level. Randomly selected 02.02.40. (JST 617112)
RO-TierllGroup 1 062.AK3.04 PBNP has no automatic interlocks between service water and CCW. Tried to write a question with a partial loss of S1 sequencer (SW pump starts/SW valves failing to open/close) and there are too many variables to get a concrete answer. This KJA doesn't match our plant, randomly re-selected.02 in AK3. (AKZ 6/12/12)
SRO-Tier 2/Group 2 028/02.02.38 Hydrogen Recombiner and purge controls are no longer covered in license basis.
Randomly selected 02.02.44. (JST 6/18112)
RO-Tier 2/Groupl 005.Al.06 Reviewed OP-7A, OP-3C, IT-03, IT-03A, TS-30 and all items related to time-to-boil which is the same item tested in RO question 16. Randomly selected Al.02. (6/19/12 AKZ)
SRO-Tier I/Group 2 005.AA2.02 Attempted several iterations of questions associated with the KJA, could not meet the requirements to be written to SRO-Ievel (system knowledge only for rod speeds).
Randomly selected AA2.04. (JST 6119112)
SRO-Tier l/Group 2 068.AA2.04 AOP-I01l0A actions under AA2 are all RO actions, SRO attachment and steps are pertaining to power, EDG's, batteries and SW. Tried writing questions but they are all RO system level or would get too complicated with plant conditions In order to maintain the 2 AA2 and 2 G questions in this tier we randomly selected a new procedure for AA2, which was 067.AA2.13 (AKZ 6/28/12)
RO-Tier 2/Group 1 012.Kl.04 PBNP has no direct cause or relationship from RPS to RPI. Questions would be related to a trip and some RP1 malfunction or a LOD I question ifwe have a trip what to the RPI's do. Randomlyreselected 08 MFW within Kl. (AKZ 6/28112)
SRO-Tier 2/Group 1 063.A2.02 Tried writing several SRO-level questions, could not tie loss of ventilation that relates to SRO-level ofknowledge. Everything kept going back to having to know only system knowledge to answer the question. Randomly selected 007.A2.03. (JST 7/27112)
SRO-Tier 2/Group 2 056/04.04.50 Tried writing 3 different SRO level questions. Due to the limited condensate system alarms we couldn't come up with an SRO only level question. Randomly re-selected a new Generic KJA 2.01.07
SRO-Tier 1/Group 2 005.AA2.04 APE is for stuck or inoperable rod and AA2.04 is KIA for dropped rod. Tried writing several questions tied to CET maps, they were RO knowledge (reading a map). It is impossible to determine if a rod is completely dropped or partially stuck just by CET maps. The additional information needed to make a TS call makes the CET map not needed. Reselected.03 under AA2. (AKZ 8/21/12)
SRO-Tier 1/Group 2 060.02.02.40 After discussion with the Chief NRC Examiner, reselected the KIA due to over sampling of generic KIA 02.02.40. Randomly res elected 02.04.18. (JST 9/24/12)
RO-Tier 1/Group 1 WE011.02.04.47 After discussion with the Chief NRC Examiner, reselected the KIA due to over sampling of generic KIA 02.02.47. Randomly reselected 02.02.37. (JST 9/24/12)
SRO-Tier 2/Group 2 028.02.02.44 After discussion with the Chief NRC Examiner, reselected the KIA due to over sampling of generic KIA 02.02.44. Per FSAR section 5.3.2.4 Post Accident Containment Venting, the NRC dropped hydrogen release associated with a LOCA from 10CFR50.44. These systems are no longer required for design basis, but are still referenced for situations beyond design basis.
Randomly reselected new system (containment iodine removal) and generic k/a 027.02.02.25. (JST 9/24/12)
RO-Tier 2/Group 2 086.02.02.38 After discussion with the ChiefNRC Examiner, reselected the KIA due to over sampling of generic KIA 02.02.38. Randomly reselected 02.04.35. (JST 9/24/12)
RO-Tier 3 02.04.09 After discussion with the Chief NRC Examiner, reselected the KIA due to over sampling of generic KIA 02.04.09. Randomly reselected 02.04.34. (JST 9/24/12)
RO-Tier 2/Group 1 003.KS.04 After 45-day submittal this question was deemed LOD 1 and GFE knowledge. Unable to get a question using S/G level due to the plant transient stabilization time. In trying to maintain the sample plan with K5, there is 4 other KIA's to choose from. 3 of the 4 are GFE type knowledge like the one we are rejecting. After discussion with the Chief Examiner we are selecting 003.K5.03. (AKZ 1112/12)
SRO-Tier 2/Group 1 076.02.04.46 Tried writing several questions and after discussion with the Chief Examiner we are re drawing another generic KIA with the same system. The current generic KIA lends itself to RO knowledge. Randomly reselected generic KIA 02.01.20.

Verify response to column H is 'YES' if no corrective action is indicated.
A. Does the test question tie directly to the learning objective(s)?
B. Is the test question at the same cognitive level as the tested objective(s)?
C. Does the lesson plan! instructor guide provide information to support the objective(s)?
D. Is the training material content correct regarding the exam answer (that is, does the material support the answer or does it lead to an incorrect answer)?
