Portable eyewash moved to conformm to DAP.IR 1389735: During the NRC task force 2.3 seismic walk down, the team identified that there was a portable eyewash station near the U2 250V DC Distribution panels that was not secured and was in close proximity to safety related equipment.

The location did not meet the distance requirements of DAP 03-20, Restraint of Portable Equipment.

DAP 03-20 Step F.3 requires the following:

IF the item will be left unattended in an area requiring seismic restraint but its aspect ratio is less than or equal to 2, THEN it shall be stored greater than 24" from safety related equipment or seismically restrained.

The eyewash station was relocated to comply with station procedures.

AWC-12 IR 1390584: The Fukushima Seismic walkdown team inspected the U2 HPCI room as part of the scope for area walk by inspections to satisfy the NRC request for information.

There is a ladder rack on the wall directly across from the doorway to the room. The ladder rack does not have any "ringers" on the ends of the rack that would keep the ladders on the rack during a seismic event. There were two ladders stored on the rack that needed to be shifted to one side to prevent the ladders from contacting seismically sensitive equipment in case of an event. There is currently no condition since the ladders have been repositioned.

8. Have you looked for and found no other seismic conditions that could adversely affect the safety functions of the equipment in the area?Evidence of prior groundwater intrusions on walls.
