Anchorage

1. Is anchorage configuration verification required (i.e., is the item one of the 50% Yes of SWEL items requiring such verification)?
2. Is the anchorage free of bent, broken, missing or loose hardware? Yes
3. Is the anchorage free of corrosion that is more than mild surface oxidation? Yes
4. Is the anchorage free of visible cracks in the concrete near the anchors? Yes
5. Is the anchorage configuration consistent with plant documentation? (Note: Yes This question only applies if the item is one of the 50% for which an anchorage configuration verification is required.)

Transmitteris mounted to two horizontal P lOOs, 10.5" apart by 4 - 3/8" bolts, i.e. one at each corner. The PlOQOs are anchoredto the concrete wall by 4 -

3/8" expansion anchors, 1 at each end. Per Dwgs J-04EC99 and J-07G 15, anchorageis consistent with plant documentation.

2. Is the anchorage free of bent, broken, missing or loose hardware? Yes Two bolts are loose at the Thomas pump behind the equipment. Doesn't look like anchoragebolts. 4 cornerbolts are secured, middle bolts hold the plate to the skid and are fully tightened. The mentioned bolts are positioningbolts (Per plantpersonnel)and this is a typical case on all Thomas pumps. Judgednot a concern.
3. Is the anchorage free of corrosion that is more than mild surface oxidation? Yes
4. Is the anchorage free of visible cracks in the concrete near the anchors? Yes
