Exposed embedded pipes, approx. 1" diameter, with rust staining. This area continues to remain unchanged since 2001 inspection. Inside of pipes filled with concrete. See photo U3-067284. Voids from unfilled form tie holes between EL 118 and El. 123 (previously identified). See photo U3-067202. Incipient spall, less than 4" in size, between El. 123 and El. 128 (previously identified). Nine locations of exposed metal embedments and rebar on the equipment hatch extension. These locations were coated in 2009 to protect against future corrosion.
