Dear Mr. Lynch:

f n a telephone conversation on September 19,2011, Mr. M' Alexander, Nine Mile Point unit 2 (NMP2) Requalification Program Lead, and Mr. T. Fish, sr. operations Engineer, made "rr"ng"rn"nts tor the U.S. NucleaiRegulatoryCommission (NRC) to inspect the licensed operator requalification program at the NMP2 facility. The inspection is pla-nned for the week of November 2g,2011, which-coincides with your regutarty scheduled requalification examination cycle. The staff at your facility should prepare and conduct the requalification examinations in atcordance with you r N Rg-approved req ual ification prog ram.

ln accordance with Tiile 10 of the Code of FederatRegulations (CFR) 55.59(c), the NRC may request facility licensees to submit their biennial comprehensive requalification written examinations or annual operating tests as necessary to support the NRC's inspection program needs. ln order for the ruifC to adequately prepare for this inspection, Rlegs.9 furnish: 1) an index or summary of condition reports written in the past two years in which licensed operator errors were determined to be the root or contributing cause; 2) an index or summary of condition reports written in the past two years on simulator performance problems; 3). a.n index or summary of simulator performance test results in the past two years; and 4)-the examination schedule, sample plan, written examinations, and operating test scheduled for the week of the Nspection to the Nic bv November 21,2011. Mr. Alexander has been advised of this request and provided with the nime and address of the NRC lead inspector assigned to this inspection.

This letter contains information collections that are subject to the Paperwork Reduction Act of lgg' (44 U.S.C. 3501 et seq.).
