IlSONGSISCE/EIX Unit 2 Train B EDG 2G003 Emergent Issues Response Team Status & Action Plan 7-12-10 Base Notification: 201009885 Inspections were conducted by FIN and HVAC. Meggar results were >1000 Megohms for both emergency fans 2MA276 and 2MA277. The motors were not able to be bridged which implies the motor rotors are spinning as result of a breeze through the fan.
HVAC reported the nose cone for fan 2MA276 was found in the air intake. A chain link fence screen encapsulates the fan nose cone, implying the screen is no longer intact. Fan 2MA276 is the western side of the building. The fan nose cone is made from aluminum.
Trouble shooting plan is approved and attached to notification 2010C" 385.
Action Plan:
Visually inspect fans 2MA276 and 2MA277.
MA276 inspected thru manway from inside of EDG building at approx 0230.
Several chunks of aluminum seen down duct downstream of fan. Some also seen on upstream side of fan where the nose cone is. Couldn't see the nose cone from this side. Fan was rotating freely, no significant damage noted on any of the blades. None of the blades were contacting the shroud.
Fan was inspected from suction side. Nose cone is missing.
All replacement parts are staged in crane bay.
A schedule is being developed that will show the correct times for the activities listed below.

C) In parallel, need to handbar 2G003 to look for loose parts. check to see air is coming from each piston (ensures all connected). After checking for damaged parts, consider running 2G003 prior to returning 2MA276 to service to ensure no problems with 2G003.
