This report summarizes the results of examinations performed during the last refueling outage (RFO-21) of the First Inservice Inspection Period of the Fourth Ten-Year Inspection Interval (December 26, 2009 through December 25, 2019).

This activity report meets the requirements of the American Society of Mechanical Engineers (ASME),

Code Case N-532-4, "Alternative Requirements to Repair and Replacement Documentation Requirements and Inservice Summary Report Preparation and Submission as Required by IWA-4000 and IWA-6000,Section XI, Division 1," as approved for use by Regulatory Guide 1.147, "Inservice Inspection Code Case Acceptability, ASME Section XI, Division 1.", Nine Mile Point Unit I Steam Dryer Support Bracket Flaw Evaluation, delineates the flaw evaluation performed for the indications identified in the NMPI steam dryer support brackets during RFO-21. Attachment 4 was prepared by Structural Integrity Associates, Inc (SAI) and includes information provided by the Electric Power Research Institute (EPRI).

ABSTRACT ASME Section XI Code Case N-532-4 requires submission of the Owner's Activity Report to the regulatory and enforcement authorities having jurisdiction at the plant site within 90 calendar days of the completion of each refueling outage.

This Owner's Activity Report summarizes the results of the Nine Mile Point Nuclear Station (NMPNS), Unit 1, American Society of Mechanical Engineers (ASME)Section XI In-service Inspection, Risk-Informed Inspection, Containment In-service Examinations, and Pressure Tests performed during Refueling Outage Twenty-One (RFO-21), from March 20, 2011 through April 20, 2011, and the First outage of the Fourth In-service Inspection Period that started on August 23, 2009 and is scheduled to end on August 22, 2012.

The attached Table I and 2 include flaws and conditions (those meeting the requirements of Code Case N-532-

4) identified since the last Owner's Activity Report following Refueling Outage RFO-20.

As an alternate to the requirements of the ASME, Boiler and Pressure Vessel Code (B&PVC),Section XI, Article IWA-6000, NMPNS is implementing ASME Code Case N-532-4, Alternative Requirements to Repair and Replacement Documentation Requirements and In-service Summary Report Preparation and Submission as Required by IWA-4000 and IWA-6000,Section XI, Division 1". Regulatory Guide 1.147, Revision 15, approved ASME Code Case N-532-4 for use. This Report complies with the requirements of Code Case N-532-4 and the conditions identified in RG 1.147.

OWNER'S ACTIVITY REPORTS - FORM OAR-1 Owner's Activity Reports Form OAR-I's were prepared and certified upon completion of the Nine Mile Point Unit 1, 2011 refueling outage (RFO-21) examination activity in accordance with the following NMPNS inspection plans and schedules:

CNG-NMPI-ISI-004, Fourth Ten-Year In-service Inspection Plan and Implementing Schedule, (includes the Alternate Risk-Informed Inspection Program CNG-NMPI-CISI-002, Second Containment In-service Inspection Plan and Implementing Schedule.

NMP1 -PT-003, Inservice Pressure Testing Program Plan In addition to the Owner's Activity Report, Form OAR-I, the following Tables are also included:

Table 2, identifies those items containing flaws or relevant conditions that exceeded Division 1 acceptance criteria shall be provided with the information and format of Table 2. This information is required even if the discovery of the flaw or relevant condition that necessitated the repair/replacement activity did not result from an examination or test required by Division 1. If the acceptance criterion for a particular item is not specified in Division 1, the provisions of IWA-3100(b) shall be used to determine which repair/replacement activities are required to be included in the abstract.

In accordance with ASME Section Xl, IWB-2412(a), IWC-2412, IWD-2412 IWE-2412(a), and IWF-2410(b) and with the exception of the examinations that may be deferred until the end of the inspection interval as specified in Table IWB-2500-1, IWC-2500-1, IWD-2500-1, IWF-2500-1 and IWE-2500-1, including those examinations which NMPNS has requested and received approval in the form of proposed alternatives in accordance with 10CFR50.55a(a)(3), the required examinations in each examination category have been completed for the First In-service Inspection Period in accordance with Tables IWB-2412-1, IWC-2412-1, IWD-2412-1, IWE-2412-1, and IWF-2410-2.

Condition Report CR-2011-003409 documents the identification of a to - Flange Weld flaw exceeding the acceptance criteria of IWB-3510. The indication is subsurface, is 1.239" in the through wall dimension, and 79" long.

The flaw has been determined to be an original fabrication defect.

A flaw evaluation was performed for the indication per the ASME Section XI (2004 Edition) acceptance criteria. The evaluation follows the similar approach used in Structural Integrity Report,

'Nine Mile Point Unit 1 RPV Closure Head Flaw Evaluation,' SIR 036, Rev. 0, July, 2003. The end of evaluation period of 60 years was used. The allowable flaw size for the circumferential flaw in the RPV closure head weld was calculated to be 1.99 inches. It is shown that the final flaw size at the end of evaluation period is still below the allowable flaw size. Thus, the indication is acceptable for continued operation.

The evaluation performed in support of this condition was submitted to the NRC for their approval on June 28, 2011.

B-N-2 B13.30 Reactor Vessel Interior Condition Reports CR-2011-002755 and CR-2011-002866 Attachment Weld -

document the identification of a relevant indication exceeding the Beyond Beltline Region acceptance criteria of IWB-3520.2. During visual examination of (Steam Dryer Support steam dryer support bracket lug (1-587C) at 230', indications were Bracket) noted in the bracket base material, right side (231 deg.) and top of the bracket lug. The indication runs for a length of 3.250" and transverses around the top right comer and continues for approximately 1.00".

Similar indications were detected in the dryer support bracket lugs at the 50 and 130 deg azimuth locations (Exam Id's 1-587A and 1-587B, respectively). Follow-up informational ultrasonic (UT) exams confirmed the indication lengths seen visually and determined the indications were relatively shallow. The UT indications are located entirely within the base metal of the bracket and run parallel to the vessel attachment weld such that they are not considered a threat to propagate into and/or through the vessel attachment weld toward the vessel.

The supporting Engineering Evaluation concluded that the indication is acceptable for continued service for at least one additional operating cycle. In accordance with IWB-3144(b) the engineering evaluation of the exam results is submitted as an attachment to this Owner's Activity Report.
