The reporting period covered by this submittal is January 1, 2010 through December 31, 2010.
Based on quantitative analysis accumulated from the drug and alcohol screening results which are recorded and maintained in Duke's Fitness-For-Duty database, this data is considered to be complete, accurate and consistent with industry and Duke program expectations.

(2) Note: Increased random screening numbers as a result of allegation issues involving drug use/abuse within the vendor groups at Oconee Nuclear Site; site management deemed accelerated random screening (at 100 percent) was warranted for the Oconee Vendor Pool for the period 10/04/10 through 11/22/10; no drug/alcohol positives were identified during this accelerated screening period.

Seventeen (17) fatigue assessments were performed - five (5) for-cause, ten (10) post-event, and two (2) self-declarations. Sixteen (16) fatigue assessments occurred during on-line work periods. One (1) self-declaration occurred during the outage.

I Analysis of Waiver Assessment Data:
Two (2) waivers were generated (See PIP M-10-1123). PT/2/A/4403/008 2B RN Flow Balance was in progress.
Two (2) Ops Test Group employees stayed over to complete the test. They had arrived at work on 2/22/10 at 0700. Work was projected to complete prior to 2/22/10 at 2300. At about 2200, after completing the test calculation, it was found that the procedure requirements were not met. Adjustments of various valves in the field would be required to adjust flow to meet flow balance conditions. This information was given to the OSM and WCC about 2230. A check of EmpCenter showed that the two (2) employees would reach 16 hours1.851852e-4 days <br />0.00444 hours <br />2.645503e-5 weeks <br />6.088e-6 months <br /> at 2300.
An assessment was performed on the two (2) employees at this time on their ability to exceed 16 hours1.851852e-4 days <br />0.00444 hours <br />2.645503e-5 weeks <br />6.088e-6 months <br />. They had been off the last two (2) days. Both stated that they felt okay to continue work.
Analysis of Fatigue Assessment Data:
Two (2) fatigue assessments were performed for the two (2) waivers. Both individuals were "Approved" per the Fatigue Assessment Form. Fourteen (14) non-waiver fatigue assessments were performed -four (4) for-cause and ten (10) post-event. There were no self-declarations.

Conclusions:==
One (1) event prompted the two (2) waivers that were generated on site. No conclusions were drawn from this single event. No actions were taken as a result of the for-cause and post-event fatigue assessments. All were given "Approved "status on their Fatigue Assessment Forms.

There were a total of twelve (12) assessments performed during the year -four (4) for-cause assessments, seven (7) post-event assessments and one (1) self-declaration. The self-declaration was a Security Officer who reported to work, but in his shift pre-job brief informed his supervisor that he had not had adequate rest during his time off and was fatigued. Eleven (11) assessments occurred during on-line periods and one (1) off-line.

No actions were taken as a result of the for-cause and post-event fatigue assessments. The Security Officer was sent home as a result of the self-declaration.
