I am forwarding a copy of the Final Report for the Biennial Plume Phase Exercise held in San Luis Obispo County on August 11, 2010, for the Diablo Canyon Power Plant (DCPP). The purpose of this exercise was to assess the level of state and local preparedness in responding to a radiological emergency. The final exercise report was prepared by the U.S. Department of Homeland Security's Federal Emergency Management Agency, Region IX Radiological Emergency Preparedness Program staff.

Based on the evaluation of the August 11, 2010, Plume Phase Exercise, the off-site radiological emergency response plans for the state of California and the affected local jurisdictions site-specific to DCPP can be implemented, and are adequate to provide reasonable assurance that appropriate measures can be taken off-site to protect the health and safety of the public in the event of a radiological emergency at DCPP.

Therefore, the Code of Federal Regulations, Title 44 Part 350 interim approval of the off-site radiological emergency response plans and preparedness for the state of California, site-specific to DCPP, will remain in effect.

I would also like to take this opportunity to acknowledge the many individuals that participated in this successful exercise. Their dedication to this program was clearly evident.

SUMMARY

The U.S. Department of Homeland Security's Federal Emergency Management Agency (FEMA)

National Preparedness Division - Technological Hazards Branch in Region IX evaluated an off-site biennial Plume Phase exercise on August 11, 2010 in the Plume Exposure Pathway Emergency Planning Zone (EPZ) around the Diablo Canyon Power Plant (DCPP). Out-of-sequence interviews were conducted on August 12, 26, and September 29, 2010. The purpose of the exercise was to assess the level of state and local preparedness in response to a radiological emergency at DCPP. This drill was held in accordance with FEMA's policies and guidance concerning the implementation of state and local Radiological Emergency Response Plans (RERP) and procedures.

The previous biennial exercise at DCPP was conducted on October 29, 2008. The qualifying emergency preparedness exercise was conducted on August 19, 1981.

FEMA wishes to acknowledge the efforts of the many individuals who participated in this exercise.

Protecting the public health and safety is the full-time job of some of the exercise participants and additional assigned responsibility for others. Still others have willingly sought this responsibility by volunteering to provide vital emergency services to their communities.

Cooperation and teamwork of all the participants were evident during this exercise.

This report contains the final evaluation of the exercise. The organizations except where noted in this report, demonstrated knowledge of their emergency response plans and procedures and adequately implemented them. There were no deficiencies or Area Requiring Corrective Action (ARCAs) identified during this exercise. No ARCAs remain uncorrected from previous exercise.

After Action Report Diablo Canyon Power Plant SECTION 2: EXERCISE DESIGN

SUMMARY

2.1 Exercise Purpose and Design FEMA Region IX evaluated the exercise on August 11, 2010 to assess the capabilities of local emergency preparedness organizations in implementing their RERPs and procedures to protect the public health and safety during a radiological emergency involving DCPP. The purpose of this report is to present the results and findings on the performance of the Off-site Response Organizations (OROs) during a simulated radiological emergency.

2.2 Exercise Objectives, Capabilities and Activities The exercise evaluation area criteria, contained in the FEMA Federal Register Notice, "Radiological Emergency Preparedness Exercise Evaluation Methodology," dated August 25, 2002 represent a functional translation of the planning standards and evaluation criteria of NUREG-0654/FEMA-REP-1, Rev. 1, "Criteria for the Preparation and Evaluation of Radiological Emergency Response Plans and Preparedness in Support of Nuclear Power Plants,"

November 1980.

The objectives of this exercise are as follows:

Criterion L.a. 1: OROs uses effective procedures to alert, notify, and mobilize emergency personnel and activate facilities in a timely manner. (NUREG-0654: A.4, D.3, 4; E.1, 2; H.4)

Criterion 1.c. 1: Key personnel with leadership roles for the ORO provide direction and control to that part of the overall response effort for which they are responsible. (NUREG-0654: A. 1.d, A.2.a, b)

Criterion 1.d. 1: At least two communication systems are available, at least one operates properly, and communication links are established and maintained with appropriate locations.

Communications capabilities are managed in support of emergency operations. (NUREG-0654:

F.1, 2)

After Action Report Diablo Canyon Power Plant Criterion 2.a. 1: OROs use a decision-making process, considering relevant factors and appropriate coordination, to ensure that an exposure control system, including the use of KI, is in place for emergency workers including provisions to authorize radiation exposure in excess of administrative limits or protective action guides. Criterion 2.b. 1: Appropriate protective action recommendations are based on available information on plant conditions, field monitoring data, and licensee and ORO dose projections, as well as knowledge of onsite and off-site environmental conditions. (NUREG-0654: 1.8, 10; Supplement 3)

Criterion 2.b.2: A decision-making process involving consideration of appropriate factors and necessary coordination is used to make protective action decisions for the general public (including the recommendation for the use of KI, if ORO policy). (NUREG-0654: J.9; J.10.f, m)

Criterion 2.c. 1: Protective action decisions are made, as appropriate, for special population groups. (NUREG-0654: J.9; J.10.d, e)

Criterion 3.a.1: The OROs issue appropriate dosimetry and procedures, and manage radiological exposure to emergency workers in accordance with the plans and procedures. Emergency workers periodically and at the end of each mission read their dosimeters and record the readings on the appropriate exposure record or chart. (NUREG-0654: K.3.a, b)

Criterion 3.b. 1: KI and appropriate instructions are available should a decision to recommend use of KI be made. Appropriate record-keeping of the administration of KI for emergency workers and institutionalized individuals is maintained. (NUREG-0654: J.10.e)

Criterion 3.c. 1: Protective action decisions are implemented for special populations other than schools within areas subject to protective action. (NUREG-0654: J. 10.c, d, g)

After Action Report Diablo Canyon Power Plant Criterion 4.a. 1: The field teams are equipped to perform field measurements of direct radiation exposure (cloud and ground shine) and to sample airborne radioiodine and particulates.

(NUREG-0654: H.10; 1.7, 8, 9).

Criterion 4.a.2: Field monitoring teams are managed to obtain sufficient information to help characterize the release and to control radiation exposure. (NUREG-0654: 1.8, 11; J. 1O.a; H. 12).

Criterion 4.a.3: Ambient radiation measurements are made and recorded at appropriate locations, and radioiodine and particulate samples are collected. Teams will move to an appropriate low background location to determine whether any significant (as specified in the plan and/or procedures) amount of radioactivity has been collected on the sampling media.

Criterion 5.a. 1: Activities associated with primary alerting and notification of the public are completed in a timely manner following the initial decision by authorized off-site emergency officials to notify the public of an emergency situation. The initial instructional message to the public must include as a minimum the elements required by current FEMA REP guidance. (10 CFR Part 50, Appendix E.IV.D & NUREG-0654: E.5, 6, 7)

Criterion 5.a.3: Activities associated with FEMA approved exception areas (where applicable) are completed within 45 minutes following the initial decision by authorized off-site emergency officials to notify the public of an emergency situation. Backup alert and notification of the public is completed within 45 minutes following the detection by the ORO of a failure of the primary alert and notification system. Criterion 5.b. 1: OROs provide accurate emergency information and instructions to the public and the news media in a timely manner. (NUREG-0654: E.5, 7; G.3.a; G.4.c) 7

After Action Report Diablo Canyon Power Plant 2.3 Scenario Summary This section contains a summary of the simulated sequence of events used as the basis for invoking emergency response actions by the OROs during the DCPP biennial exercise on August 11, 2010, submitted to FEMA by the state of California and DCPP.

An air compressor breaks apart causing parts to fly about the room and oil to splatter, resulting in a fire in an emergency diesel generator room. The control room receives a fire alarm and a fire suppression Cardox system actuation alarm. The fire is not immediately suppressed by the Cardox actuation because the West doors do not close as designed. Since the fire impacts safety related equipment in a vital area, the shift manager declares an Alert Emergency Level Classification (ECL). DCPP initiates notifications to the San Luis Obispo County Watch Commander and the California Emergency Management Agency of the Alert ECL.

Approximately forty minutes later, a high stator temperature on Centrifugal Charging Pump 1-3 causes the pump to trip. The related Reactor Coolant Pump (RCP) 1-3 suffers a locked rotor failure that results in a reactor trip signal. The automatic trip fails to shut down the reactor and manual actions taken from the reactor control console are not successful in shutting down the reactor. As a result, the reactor fuel is significantly damaged, justifying a Site Area Emergency (SAE) ECL. A weld in RCP 1-3 fails, resulting in a reactor coolant system leak rate greater than the available makeup capacity. Increased radiation levels detected by the containment high range radiation monitors indicate a potential loss of the containment barrier. The General Emergency ECL is met with the loss of any two barriers and loss or potential loss of a third barrier.

After Action Report Diablo Canyon Power Plant SECTION 3: ANALYSIS OF CAPABILITIES 3.1 Exercise Evaluation and Results Contained in this section are the results and findings of the evaluation of all jurisdictions and functional entities that participated in the August 11, 2010 biennial Plume Phase exercise to test the off-site emergency response capabilities of state and local governments in the EPZ surrounding DCPP. Each jurisdiction and functional entity was evaluated on the basis of its demonstration of criteria delineated in the exercise evaluation areas as outlined in the August 2002, Interim REP Program Manual. Detailed information on the extent-of-play agreement used in this exercise is found in Appendix C of this report.

3.2 Summary Results of Exercise Evaluation The matrix illustrated in Table 3.1, presents the status of all exercise evaluation area criteria that were scheduled for demonstration during this exercise by all participating jurisdictions and functional entities. Exercise criteria are listed by number and the demonstration status is indicated by the use of the following letters:

M - Met (No Deficiency or ARCAs assessed and no unresolved ARCAs from prior exercises)

D - Deficiency assessed A - ARCAs assessed or unresolved ARCAs from prior exercises N - Not Demonstrated (Reason explained in Appendix D, extent-of-play agreement)

Presented below are definitions of the terms used in this section relative to criteria demonstration status.

" Met - Listing of the demonstrated exercise evaluation area criteria under which no Deficiencies or ARCAs were assessed during this exercise and under which no ARCAs assessed during prior exercises remain unresolved.

" Deficiency - Listing of the demonstrated exercise evaluation area criteria under which one or more Deficiencies were assessed during this exercise. Included is a description of each Deficiency and recommended corrective actions.

" Area Requiring Corrective Action - Listing of the demonstrated exercise evaluation area criteria under which one or more ARCAs were assessed during the current exercise.

The following are classifications of the three types of issues that are discussed in this report:

A Deficiency is defined in the FEMA Interim REP Program Manual, August 2002 as "...an observed or identified inadequacy of organizational performance in an exercise that could cause a finding that off-site emergency preparedness is not adequate to provide reasonable assurance that appropriate protective measures can be taken in the event of a radiological emergency to protect the health and safety of the public living in the vicinity of a nuclear power plant."

An ARCA is defined in the FEMA Interim REP Program Manual, August 2002 as "...an observed or identified inadequacy of organizational performance in an exercise that is not considered, by itself, to adversely impact public health and safety."

FEMA has developed a standardized system for numbering issues. This system is used to achieve consistency in numbering exercise issues among FEMA Regions and site specific exercise reports within each Region. It is also used to expedite tracking of exercise issues on a nationwide basis.

The identifying number for Deficiencies and ARCAs includes the following elements, with each element separated by a hyphen (-).

" Plant Site Identifier - A two-digit number corresponding to the Utility Billable Plant Site Codes.

" Exercise Year - The last two digits of the year the exercise was conducted.

" Evaluation Area Criterion - A letter and number corresponding to the criteria in the FEMA REP Exercise Evaluation Methodology.

CORRECTIVE ACTION DEMONSTRATED: At the designated time, the air sampler was stopped. An ambient radiation reading was taken to determine if the plume had shifted during the sampling time. Procedure 111.06 HP-3, Checklist 5 has been revised, and now instructs the FMTs to "measure post air sample general area dose rates with the ASP-1 (open and closed window)."

After Action Report Diablo Canyon Power Plant ISSUE NO.: 19-08-4a3-P-2 ISSUE: The Field Monitoring Procedure, 111.06 HIP-3 Checklist 5, gives no guidance for contamination control while handling samples. Air particulate filters and charcoal canisters may become contaminated during handling while being prepared for field counting and transfer to the OEL.

CORRECTIVE ACTION DEMONSTRATED: A muslin cloth was placed on the ground and the counting jig was placed on the cloth to provide a clean work area.

Gloves were donned to prevent any spread of contamination. The particulate filter was removed from the envelope with tweezers and placed on the mesh within the jig.

The Eberline E-140 with attached pancake probe was used to count the particulate filter. The net counts were at background. The filter was removed from the jig with the tweezers and returned to the sample envelope. After counting the filter, the cartridge was removed from the labeled bag and the same process was repeated with the cartridge being inserted into the jig below the mesh. The net count was again at background.

ISSUE NO.: 19-08-4a3-P-3 ISSUE: The FMTs use charcoal cartridges for the collection of radioiodine. SOP 111.06 HP-3, Checklist 5 indicates that a TEDA impregnated activated charcoal cartridge is used for the collection of radioiodine. The FMT's equipment and supply kits had 20 charcoal cartridges for this task. Once a sample was collected, the cartridge was counted using an Eberline E-140 N count rate meter with an HP-210 pancake probe. This practice is not consistent with industry standards and will result in overestimating the quantity of radio iodine present.

CORRECTIVE ACTION DEMONSTRATED: The FMTs are now equipped with two tubes often Radioiodine Sampler Model GY-130 Silver Zeolite cartridges with an expiration date of July 2019. A tube often TEDA impregnated charcoal cartridges are also provided for exercise and drill purposes. SOP 111.06 HP-3 was also revised to indicate that silver zeolite cartridges will be used to collect air samples for real events.

They either had an expiration date of March 2007, or March 2008 printed on each of the blister packs. A letter from the Department of Health and Human Services was mentioned, and was later made available which was interpreted as extending the expiration date, though the letter was confusing as it did not specifically address a manufacturer or product lot number.

CORRECTIVE ACTION DEMONSTRATED: All KI packages have been replaced in the Emergency Worker Exposure Control (EWEC) kits at all evaluated locations, and now have an expiration date of April 2016. In August 2009, the San Luis Obispo County Public Health Department also sent out a mailer informing all members of the public who reside or work within the EPZ that the KI tablets distributed in 2003 would be expiring. A voucher was attached to the mailer, listing pre-distribution locations to allow for members of the public to receive KI tablets.

After Action Report Diablo Canyon Power Plant SECTION 4: CONCLUSION A biennial exercise was conducted on August 11, 2010 to assess the capabilities of state and local emergency preparedness organizations in implementing their RERPs and procedures to protect the public health and safety during a radiological emergency involving DCPP. The purpose of this exercise report is to present the exercise results and findings on the performance of the OROs during a simulated radiological emergency.

The findings presented in this report are based on the evaluations of the federal evaluation team, with final determinations made by the FEMA Region IX Regional Assistance Committee Chairperson and approved by the Regional Administrator.

No deficiencies and no ARCAs were identified during this exercise for the state of California or San Luis Obispo County.

Based on the evaluation of the August 11, 2010 biennial exercise and subsequent out-of-sequence interviews, the off-site radiological emergency response plans for the state of California and the affected local jurisdictions site-specific to DCPP can be implemented, and are adequate to provide reasonable assurance that appropriate measures can be taken off-site to protect the health and safety of the public in the event of a radiological emergency at DCPP.

PURPOSE, SCOPE & GEELRAL CONCE1I'S This extent-of-play agreement addresses participation by State of California agencics and local jurisdictions in the Diablo Canyon Power Plant (DCPP) evaltuated-exercise scheduled for August II, 2( 10. In order to show continuity among participating agencies, this agreement includes the extent of play by:

State of Calilbmia agencies, including the California Emergency Management Agency (CalEMA) and the California Departmeint of Public Health (CDPH).

Locally-based State of California agencies, including Calif. Highway Patrol (CIHP)), Calif.

State Parks, and Calif. Dept. ofTransportalion (Caltrans).

Local agencies and jurisdictions, including the County of San Luis Obispo (SIO Co.) and participating Departmcnts and Agencies and Cities within the County.

The purpose of this extent-of-play agreement is to present the evaluation areas that will be demonstrated and to idcntify planned deviations from the implementation of the applicable plans. including the State ofCalifornia Nuclear Power Plant Emergency Response Plan (State NPP Plan., 2008), the CDPH Nuclear Power Plant Emergency Response Procedures (which are a pairt of the State NPP Plan): and San Luis Obispo County Nuclear Power Plant Emergency Response Plans and Procedures. The state and local agencies will implement applicable elements in these plans and procedures unless otherwise specified in this agreement.

RF-I)ENIONSTRATION AND IN-PLAV INTERVIEWS Areas requiring corrective action may be immediately corrected and re-evaluated during tile exercise or inumediately following, providing that the re-demonstration is not disruptive, does not interrupt the flow of the exercise, and does not affect other evaluation areas. During demonstrations conducted out-of-sequence, the Evaluator may have the participants re-demonstrate an activity that is determined to be not satisfactorily demonstrated. Inumediate training prior to the re~demonistration may be allowed, again provided that ilhetraining and re-demonstration arenot disruptive, do not intemipt the flow of the exercise, and do not affect other evaluation areas. If the re-demonstration is performed correctly. the Evaluator will document the isstue of concern, with a statement that the issue was corrected during tile exercise. If the demonstration is still perfonned unsuccessfully, the issue will be documented and referred to the RAC Chair to determine the appropriate classification of the issue.

Evaluation areas that.are not demonstrated due to scenario play may be done through interview during or immediately following the exercise. An evaluator may interview players to determine sequence of events or actions thatwould have been taken if tile scenario had been different. Evaluator interview of players should not be disruptive nor interrupt flow of the extercise.

4. OUTSTANDINC ISSUES The last FEMA-evaluated plume-phase exercise at l)iablo Canyon Power Plant was held on October 29, 2008. There were no deficiencies and three Areas Requiring Corrective Action (ARCA)'identified during the exercise. Two ARCAs were corrected during the exercise.

ARCA 19-08-5a3-A-03 will be resolved on August Ii, 2010 upon receipt of updated siren coverage and route alerting maps pre-deployed to fire stations in the EPZ.

5. EXTENT OF PLAV 5.1 Ovierview of Extent of Play The following Extent of Play (EOP) has been developed with state and local jurisdictions involved in the planning and response to Diablo Canyon Power Plant erhergencies. This EOP represents the level of participation and any limitations oii the demonstration of Emergency Preparedness Evaluation Area Elements by the participating agencies and brganizations. 'The Emergency Preparedness Evaluation Areas

- the elements and sub-elements - for this exercise are those that are required to be demonstrated in every exercise, as required by the Interim REP Program Manual

'(Auigust 2002).' Copies of applicable plans and procedures have been provided to FEMA, Region IN staff.

5.2 NRC Participation Nuclear Regulatory Commission Region IV will not participate in the Diablo Canyon Power Plant exercise during the 2010 evaluated exercise.

  • Participation of the CDPH Department Operations Center (DOC) will be simulated. The CDPH DOC. will not be activated or evaluated during this exercise. The primary responsibilities oflthisfacility during the emergency phase are to dispatch CDPH personnel, to establish communications with the JIC. to receive informiation from staff at the UDAC and JIC, and provide support, if needed. Response to requests from CDPH involved in the exercise will be simulated to facilitate the conduct of the exercise. The simulation player(s) will be located in Sacramriento.
