Perry Nuclear Power Plant Docket No. 50-440, License No. NPF-58 NRC Initial License Examination, Perrv License Class 2008-01 There were two examination security issues encountered in the NRC Initial Reactor Operator license examination for Perry License Class 2008-01.

Issue 1: During the NRC Exam Prep week (May 25-27,2010),a speaker box, located in a communications equipment room adjacent to the simulator control room, was discovered to be broadcasting simulator radio transmissions. A member of the Simulator Engineering Group, who was on the Exam Security Agreement, heard the broadcasts in an adjacent restroom and in the hallway outside the communications room. This condition caused a potential loss of examination security.

Issue 2: On June 21, 2010, while the NRC written examination was being administered, a sheet of paper containing one test question for the written examination was found in a copy machine located in the Training and Education (TEC) building. The sheet was retrieved and provided to a trainer on the Exam Security Agreement. This condition caused a potential loss of examination security.

These issues were entered in the corrective action program for investigation under Condition Report (CR) 10-78636,Potential NRC Initial License Exam Simulator Security Violation and Condition Report 10-78594, Potential NRC Written Exam Compromise. The Perry Training staff performed the investigation for CR 10-78636.

The FENOC Fleet Training staff performed the investigation for CR 10-78594. The investigation results including causes and corrective actions were provided to you separately.

Perry Nuclear Power Plant L-10-224 Page 2 of 2 The simulator scenario associated with the broadcast transmissions was removed from the operating exam; a spare scenario was substituted. The test question associated with the copy machine was determined to not be an exam compromise and was retained in the written exam.

The CR corrective actions to be applied are considered sufficient to preclude future similar challenges to examination security.

The simulator instructors who administered previous license examinations were interviewed regarding the speaker box. None of the instructors indicated that they had heard simulator scenario transmissions outside the simulator envelope in the past.

Based largely on the interview responses, it is believed that no issues, caused by presence of the speaker box, existed with examination security during previous reactor operator initial license and requalification simulator examinations.
