Root cause investigation performed as a result of the NRC finding
- Inadequate Change Management - procedure change that revised the Explosion Definition was not communicated effectively
- Less than adequate communications - Security personnel who witnessed the switchyard explosion did not effectively communicate the information to the Control Room
Corrective Actions
- Training for Security on EALs, interface with Operations, and the importance of precise communications
- Revised Emergency Response continuing training to include off-normal procedures
- Revised Emergency Classification procedure to provide direct tie to off-normal procedures
