June 25, 2009 - Switchyard Event Inadequate Emergency Classification Root cause investigation performed as a result of the NRC finding

- Inadequate Change Management - procedure change that revised the Explosion Definition was not communicated effectively

- Less than adequate communications - Security personnel who witnessed the switchyard explosion did not effectively communicate the information to the Control Room Corrective Actions
