Palo Verde 3 Steam generator Inspection Report dated 11/18/09 For the subject SG inspection report, our staff had a couple of questions:
- 1. Was FOSAR the only secondary side inspection performed during U3R14? If not, what other secondary side inspections were performed?
FOSAR was the only inspection performed during U3R14 on the secondary side.
- 2. On the bottom of Table 1 of the November 18, 2009 submittal, there were 4 rows listing rotating probe inspections. The purpose of these inspections was not clear. Were these inspections a follow up to bobbin coil inspections (e.g., I-codes), or were they a part of a more defined sampling program (e.g., dents and dings, low row U-bends)? If the latter, briefly describe the scope.
These are +point examinations conducted to follow up bobbin coil examination results.
They include the typical bobbin I-codes, wear, MBMs, Dents, PLPs, etc. The table and scope are divided up into the 4 rows due to examinations being conducted from the hot or cold legs, and with u-bend (flexible +point) or straight (solid body +point) probes.
Row 1 thru 4 short radius u-bends were +point examined this outage and itemized on Table 1 as follows:
ROW 1 THRU 4 SHORT RADIUS U-BENDS

You can send us an informal e-mail response, or we can have a phone call if needed. While this is not a high priority action, the questions seem straightforward, so I'd like a response by February 12, if possible.
