Palo Verde Nuclear Generating Station (PVNGS)

Units 1, 2 and 3 Docket Nos. STN 50-528/529/530 Senior Operator License Application Arizona Public Service (APS) requests a waiver of the written examination and operating test requirements for a Senior Reactor Operator as described in 10 CFR 55.47 and NUREG 1021, ES-204, paragraph D "Waiver Criteria." This letter, and , documents the completion of "Additional Training" for Mr. Mark A. Sharp.

Mr. Sharp's license was terminated December 11, 2006, upon resigning from APS. He had been away from the requalification program from November 2006 to June 2008.

Mr. Sharp completed a training process of self-study and one-on-one instruction, was exposed to the license operator requalification material included in all 5 cycles of 2007, and the material for cycles 1 and 2 of 2008. He recommenced attending license operator requalification in cycle 3 (June) of 2008.

At the completion of this additional training, Mr. Sharp took and passed the scheduled annual licensed operator requalification written and simulator operating exams administered September 17 and 18, 2008, and Job Performance Measure exams administered October 21 and 22, 2008. Additionally, Mr. Sharp has spent a total of 104 hours0.0012 days <br />0.0289 hours <br />1.719577e-4 weeks <br />3.9572e-5 months <br /> on shift with the following breakdown: 1) 20 hours2.314815e-4 days <br />0.00556 hours <br />3.306878e-5 weeks <br />7.61e-6 months <br /> as a non-licensed operator under the direction of the qualified on duty operators performing plant walk downs and refamiliarization tours; 2) 36 hours4.166667e-4 days <br />0.01 hours <br />5.952381e-5 weeks <br />1.3698e-5 months <br /> of shift functions under the direction of a reactor operator in the position of Reactor Operator, and; 3) 48 hours5.555556e-4 days <br />0.0133 hours <br />7.936508e-5 weeks <br />1.8264e-5 months <br /> of shift functions under the direction of a senior reactor operator in the position of Control Room Supervisor.

U.S. Nuclear Regulatory Commission Senior Operator License Application Page 2 Pursuant to the provisions of 10 CFR 55.31 and NUREG-1 021, Revision 9, Supplement 1, APS is submitting herein the senior reactor operator license application for the individual identified above. Enclosure 1 provides an original and two copies of the NRC Form 398, "Personal Qualification Statement - Licensee," and a copy of NRC Form 396, "Certification of Medical Examination by Facility Licensee," for the applicant.

APS requests that the information provided in Enclosure 1 be withheld from public disclosure pursuant to 10 CFR 2.390(a)(6) and 10 CFR 9.17(a)(6).

There are no commitments made to the NRC by this letter.
