ATTACHMENT REPORT OF DAMAGED TRITIUM EXIT SIGN Nine Mile Point Nuclear Station, LLC November 17, 2008

L ATTACHMENT REPORT OF DAMAGED TRITIUM EXIT SIGN A. BRIEF DESCRIPTION OF THE EVENT On October 17, 2008, a material handler damaged a Tritium Exit Sign (TES) while operating a forklift in the warehouse at Nine Mile Point Nuclear Station. The warehouse is located inside the Owner Controlled Area but outside the Protected Area. This incident was documented in the site Corrective Action Program.

B. REMEDIAL ACTION The damaged TES was manufactured in June 1998. The sign had tubes containing tritium on both sides. Only one side of the sign was partially damaged. It is estimated that 2.9 curies of tritium escaped due to the damage.

Nine Mile Point Nuclear Station, LLC (NMPNS) conducted a radiological survey, quarantined the damaged TES for shipping to the manufacturer, took smear samples and also took urine samples from two employees who came in contact with the damaged TES. Bioassay data of the urine samples indicate that there is no recordable dose for the two employees who came in contact with the damaged TES.

NMPNS plans to ensure the area is clean through decontamination and surveys.

C. SHIPPING The damaged TES will be shipped to the manufacturer of the TES. A separate report will be sent to the NRC within 30 days of the shipment, as required by 10 CFR 31.5(c)(8)(ii).
