MID-CYCLE PERFORMANCE REVIEW AND INSPECTION PLAN -
FERMI POWER PLANT, UNIT 2
Dear Mr. Davis:
On August 14, 2008, the NRC staff completed its performance review of the Fermi Power Plant, Unit 2, for the first half of the calendar year 2008 assessment cycle. Our technical staff reviewed performance indicators (PIs) for the most recent quarter and inspection results over the previous 12 months. The purpose of this letter is to inform you of our assessment of your safety performance during this period and our plans for future inspections at your facility.
This performance review and enclosed inspection plan do not include security information. A separate letter designated and marked as "Official Use Only - Security Related Information" will include the Security Cornerstone review and resultant inspection plan.
Plant performance for the most recent quarter at Fermi Unit 2 was within the Licensee Response column of the NRC=s Action Matrix, based on all inspection findings being classified as having very low safety significance (Green) and all PIs indicating performance at a level requiring no additional NRC oversight (Green). Therefore, we plan to conduct reactor oversight process (ROP) baseline inspections at your facility.
In our 2007 end-of-cycle assessment letter, dated March 3, 2008, (ADAMS Ref. ML080600499) we advised you of a substantive cross-cutting issue in the area of Human Performance, based on the number of findings with a common cross-cutting aspect of procedural adequacy (H.2(c))
and procedural adherence (H.4(b)). During this period, the number of findings with the same cross-cutting aspects was not reduced and our substantive cross-cutting issue criteria in IMC 0305 were met. You have initiated actions to improve performance, however, the scope and progress of your efforts in addressing the cross-cutting performance deficiencies has not been sufficient to provide adequate confidence in your effectiveness in resolving this issue.
Therefore, this cross-cutting issue will remain open until the NRC gains confidence in your efforts and progress in addressing human performance errors. In addition, because you have a substantive cross-cutting issue in the area of Human Performance for two consecutive assessment cycles, we are requesting that you provide a written response to the issue to include a discussion of your actions undertaken, those actions planned to address the concern, and a current assessment of performance. This response should be sent to the NRC within
J. Davis 30 days of receipt of this letter. As an alternative to a written response, should you want to meet with us to discuss human performance at Fermi 2, a public meeting will be accommodated at an agreed upon time and location.
The enclosed inspection plan details the inspections, less those related to physical protection scheduled through December 31, 2009. The inspection plan is provided to allow for the resolution of any scheduling conflicts and personnel availability issues well in advance of inspector arrival onsite. Routine resident inspections are not listed due to their ongoing and continuous nature. The inspections in the last nine months of the inspection plan are tentative and may be revised at the end-of-cycle review.
In accordance with 10 CFR 2.390 of the NRC=s ARules of Practice,@ a copy of this letter and its enclosure will be made available electronically for public inspection in the NRC Public Document Room or from the Publicly Available Records (PARS) component of NRC's document system (ADAMS). ADAMS is accessible from the NRC Web site at http://www.nrc.gov/reading-rm/adams.html (the Public Electronic Reading Room).
