(ENO) is providing the enclosed steam generator tube inspection report in accordance with Technical Specification 5.6.8. ENO is required to submit the report within 180 days after initial entry into mode 4 following completion of the inspection. Mode 4 was entered on October 14, 2007.

Summary of Commitments This letter contains no new commitments and no revisions to existing commitments.

Inspection Summary This report provides the Technical Specifications (TS) section 5.6.8, Steam Generator [SG] Tube Inspection Report, required information.

The scope of inspections performed on each SG (Table 1),

Active degradation mechanisms found (Table 2),

Nondestructive examination techniques utilized for each degradation mechanism (Table 3),

The TS section 5.5.8b.1 steam generator tube integrity structural integrity performance criterion was satisfied. The TS section 5.5.8b.2 steam generator tube integrity accident induced leakage performance criterion was satisfied.

Leakage did not exceed the 0.3 gallon per minute (gpm) limit.

Primary-to-secondary leak rate measured using condenser off-gas (Xenon-1 33 or Xenon-1

35) was <0.001 gpm for operating cycle 19 with a slightly decreasing trend over the cycle.

No leakage was predicted for the 2006 to 2007 operating cycle due to observed SG tube degradation at the 2006 refueling outage. All plugs installed in the Palisades SGs post-operation are Westinghouse ribbed mechanical plugs, which are leak-tight for all plant conditions. The Palisades SGs also contain 308 tubes plugged pre-service in SG E-50A, and 309 tubes plugged pre-service in SG E-50B. Of the tubes plugged pre-service, one tube in SG E-50A and four tubes in SG E-50B are plugged using welded plugs, which are leak tight. The remaining tubes plugged pre-service are plugged using Combustion Engineering (C-E) roll plugs, which are also designed to be leak tight for all plant conditions.

Fourteen tubes were plugged in SG E-50A and ten tubes in SG E-50B during the 2007 refueling outage.

Refer to Tables 5 and 6 for details. No tubes were pulled in either SG. No in-situ pressure testing was done on any tubes.

Condition Description SG E-50A tube R136 C77 was examined with a +Pt rotating pancake coil (RPC) at the cold leg diagonal bar (DBC) location to confirm the morphology of a historical wear scar at the upper edge of DBC. The +Pt coil data analysis confirmed the bobbin signal as tube wear due to structure interaction, however, an additional volumetric signal was reported. This signal was located immediately between the two diagonal bars on either side of the tube. The 4

signal was approximately three inches long, with a maximum estimated depth of 16% through wall (TW) using the +Pt coil and sizing procedure as defined by EPRI Technique Specification Sheet (ETSS) 96910.1. The signal extended below the DBC by approximately one inch.

It was suspected that the source of the signal was tube wear due to interaction with an adjacent tube in the same column.

Only tubes in the same column could interact as the diagonal bars prevent interaction in the lateral direction.

R134 C77 (the inboard tube) was inspected with +Pt RPC; no degradation was observed.

R138 C77 (the outboard tube) was found to have been plugged in 1998 due to a 39% TW indication at vertical strap (VS7) and a 16%TW indication at the DBC. As +Pt RPC testing of tube R138 C77 could not be performed, potential tube to tube contact with wear could not be ruled out as a source of the indication. Tube R138 C77 had been preventively stabilized using a 384-inch stabilizer and plugged.

Analysis of the bobbin data of R136 C77 could not identify the additional indication.

This could be attributed to the shallow taper of the indication which would have a minimal influence upon the differential channels. The absolute channels were also examined but no flaw-like signal was present. The axial location (2/3 of the flaw length inside of the diagonal bar) could also add an additional interference with regard to detection of such indications. As a precaution, the bobbin data, in particular the absolute channels, was reviewed for all tubes in rows 130 through 138. No abnormal signals or characteristics were noted. The RPC data (from all previous inspections) for tubes with bobbin reported wear scars at the DBC and hot leg diagonal bar (DBH) in rows 134 through 138 were reviewed for both SGs. No similar indications were reported.

In addition, the bobbin based signal elevation reports for all indications at the DBC or the DBH were reviewed to identify any indication which might extend beyond the edge of a diagonal bar. Three locations were reported to have bobbin signals slightly outside of the normal elevation range of +/- 2 inches about the structure. The RPC data for these locations was also reviewed; the only wear signals reported were located on the diagonal bars. As the row count decreases the distance between the diagonal bar region and uppermost eggcrate also decreases. The eggcrate controls the tube positioning, thus the potential to experience a similar situation is decreased with decreasing row count.

Corrective Action:

The R136 C77 tube was plugged following the insertion of a 384-inch stabilizer (WO 284927). An extent of condition evaluation identified no additional tubes with this condition.

This new degradation mechanism inspection using +Pt RPC and technique ETSS 96910.1 was identified in the 2007 Steam Generator Degradation Assessment as an active damage mechanism.

Additionally, this active degradation mechanism was discussed in the 2007 Steam Generator Condition Monitoring and Operational Assessments. This active damage mechanism will be included in the 2009 Steam Generator Degradation 5

Assessment. The additional tubes to be +Pt RPC eddy current tested are included in the special interest scope.

Secondary Inspection in the SGs included Annulus and Tube Lane Foreign Object Search and Retrieval (FOSAR)

The possible loose parts in both SGs were reviewed by FOSAR. No loose parts required removal in SG E-50A.

Only small sludge rocks, that broke when retrieval was attempted, were identified.

In SG E-50B "two" loose parts were identified and removed.

Several small pieces of demister wire, -16 mils diameter X <_1l2-inch lengths, from the moisture separator reheaters, are referred to as one loose part. The other loose part was a piece of carbon steel -1/4" x 3" The remaining loose parts were sludge rocks that broke during retrieval attempts.

A visual inspection of the lower batwings (diagonal bars) was completed for both SGs. No broken batwings were identified.

There were four tubes plugged as a result of interaction with a possible loose part (PLP). Three tubes were plugged in SG E-50A and one in SG E-50B SG E-50A: R129 C108, just above cold eggcrate 08C.

This location is one tube pitch in from periphery. PLP signals are reported for both 8129 C108 and R128 C109; only R129 C108 has a single volumetric indication (SVI).

Both tubes removed from service by tube plugging. Tube R129 C108 had a 368-inch stabilizer installed prior to plugging.

SG E-50A, R136 C77 at the diagonal cold bar (DBC).

This location was RPC tested because of a bobbin wear call at 08C + 1.84 inches. As discussed above, this new damage mechanism wear scar was confirmed, and is on one of the diagonal bars. Tube R136 C77 had a 368-inch stabilizer installed prior to tube plugging.

SG E-50B : R70 C13 just outside of vertical strap 4 (VS4).

This is a peripheral location. No PLP reports for surrounding tubes; RPC of surrounding tubes has been completed. Tube R70 C13 had a 368-inch stabilizer installed prior to tube plugging The three SG tubes with single volumetric indications (SVI) were stabilized and removed from service by tube plugging in both steam generators.

There was one additional tube plugged (due to a PLP) next to a single volumetric indication, 6
