All accessible interior surfaces above the waterline were observed from this vantage point.

Notes:

Bay 3 - Inner Torus wall. There are (3) three areas of coating and primer loss with light corrosion approximately 6 above the water line in the coating that was replaced in 1998.

1. One is approximately 18 from the Bay 2 Ring Girder and is approximately 2 x 2.
2. The second is approximately 4 from the Bay 2 Ring Girder and is approximately 1 x 2.
3. The third is approximately 5 from the Bay 2 Ring Girder and is approximately 4 x 4.

The extent of material loss in these areas is indeterminate visually due to the distance (approximately 15) that the indications must be viewed from. An ultrasonic examination was performed from the Torus OD and no material loss was identified. See Ultrasonic Thickness data sheet # 24-004902-000.

THESE THICKNESS READINGS WERE TAKEN ON THE INNER RING, OD SURFACE OF TORUS BAY 3 AS A RESULT OF AREAS NOTED DURING GENERAL VISUAL EXAMINATION OF THE INTERIOR OF THE TORUS DURING RFO-24. THE LOCATIONS ON THE INTERIOR WALL WERE TRANSPOSED TO THE EXTERIOR AND THE THICKNESSES WERE TAKEN THERE. THERE ARE NO INDICATIONS OF PllTlNG OR MATERIAL LOSS IN THE IDENTIFIED AREAS. The nominal thickness in the area of interest is.533. The lowest reading found is,597.

THE WATER LEVEL IN THE TORUS WAS rDENTlFlED BY THE OPERATIONS DEPARTMENT AS BEING 11.42 WHEN THE INDICATIONS WERE RECORDED. THICKNESS READINGS WERE TAKEN IN THE LOCATIONS AS SHOWN ON PAGE 2.

Area of interest 1 is 12 by 12; area of interest 2 is 12 by 36. Both areas are gridded in 2 grids. Each grid was scanned entirely and the lowest reading was recorded.

Note 1: Rust staining previously identified in IDR 99-018 in the areas of penetrations X-5G and X-5H is caused by leakage from the Containment Spray header nozzles on elevation 252. The staining is from rusted conduit and unistrut that has run down the wall to elevation 238. The coating in the area of the rust stain is intact. There are general areas of localized missing coating where the primer is intact. This staining is not recent as evidenced by no staining appearing on the Drywell Moisture Barrier which was replaced in 2001 via MM 2000-010.

1. There are (2) two areas where the coating and primer is missing and the underlying metal is corroded.

They are located at approximately the 7 5 O azimuth just below the 251 floor elevation dust to the right of the equipment hatch and penetration X-6). Both areas are approximately 2 long by I wide (see photograph below). This condition Is caused by leakage from the Containment Spray header nozzles on elevation 252. The staining is from rusted conduit and unistrut that has run down the wall.

There are (2) two blisters located immediately to the left of the indications noted above. One is approximately 3 x 4 the other is approximately 1 x 2 (see attached photographs).

These conditions are acceptable in accordance with the criteria of Engineering Standard ENN-EP-S-001 WO, Sections 5.2,5.3 and 5.4. Also, the indications were directly observed by J. Fitzpatrick of the Mechanical/StructuraI Engineering group and determined to be acceptable as-is.

The Mechanical/StructuraI Engineering group has requested that these areas be examined again during RFO-25.

obstructed between the 269 elevation floor to the 275 elevation. From 22 clockwise from the 1802 azimuth for approximately 42 cfockwise the containment liner is 100%

obstructed from the 269 elevation floor to the bottom of the monorail track.

2.. From the 1809 azimuth counter clockwise approximately 18 the, containment liner is 100% obstructed between the 269 elevation floor to the 275 elevation. From 18 counter clockwise from the 180s azimuth for approximately 48 counter clockwise the containment liner is 100% obstructed from the 269 elevation floor to the bottom of the mono rai I track.

Exterior

The lower portion of the outer surface has general small (I to 2) areas of missing paint. Most appear to be the result of bolting removal and installation.. Theprimer is intact and there is no evidence of corrosion.

The upper portion of the outer surface has broad areas of paint checking and missing paint. There is no evidence of corrosion staining in the areas of paint checking. There are also general areas of missing primer with light corrosion. There is no evidence of material loss in the areas where the primer is missing.

The conditions listed above are acceptable in accordance with Engineering Standard ENN-EP-5-001, Revision 0, Sections 5.2, 5.3 and 5.4.

Penetration X-3 (Drywell Flange) oustructions:

The interior of penetration X-3 is obstructed by Containment Cooling duct work. Most of the circumference is obstructed except for an approximately (6) six foot length where there is no duct work. There is also a gap between the duct work and the reactor vessel shell on the reactor vessel side where approximately (1) one foot of the surface of penetration X-3 can be seen. None of the surface directly under the duct work can be seen.
