FERMI POWER PLANT - NOTIFICATION OF AN NRC BIENNIAL HEAT SINK PERFORMANCE INSPECTION AND INFORMATION REQUEST

Dear Mr. Cobb:

On March 19, 2007, the NRC will begin the Biennial Heat Sink Performance Inspection at your Fermi Power Plant. This inspection will be performed in accordance with NRC baseline inspection procedure 71111.07. The systems selected for detailed review during this baseline inspection are the Emergency Equipment Cooling Water (EECW) Heat Exchanger (Division 1),

the HPCI Room Cooler, and the RCIC Room Cooler.

We have enclosed requests for the documents needed for this inspection.

This information can be sent to the following e-mail address dls3@nrc.gov or mailed on a disk.

Please send the information by February 26, 2007, so we may start the review of these documents. A hard-copy with the required information is also an acceptable option.

The lead inspector for this inspection is Mr. Darrell Schrum. If there are questions about the material requested, or the inspection, please call (630) 829-9741.

In accordance with 10 CFR 2.390 of the NRC's "Rules of Practice," a copy of this letter and its enclosure will be available electronically for public inspection in the NRC Public Document Room, or from the Publicly Available Records (PARS) component of NRC's document system

BIENNIAL HEAT SINK INSPECTION DOCUMENT REQUEST Inspection Dates: March 19 through 23, 2007 Inspection Procedure: IP 71111.07 Heat Sink Performance Lead Inspector: D. Schrum (630) 829-9741, dls3@nrc.gov Information Requested Prior to the Information Gathering Visit The systems selected for detailed review during this baseline inspection are the Emergency Equipment Cooling Water (EECW) Heat Exchangers (Division 1), the HPCI Room Cooler, and the RCIC Room Cooler. Unless otherwise specified, all the below requests are for the selected heat exchangers (HXs).

1) Copies of the two most recent completed tests confirming thermal performance for those heat exchangers which are performance tested. In addition, for the most recently completed test confirming thermal performance of the sample heat exchangers, please provide documentation and procedures that identify the types, accuracy, and location of any special instrumentation used for these tests, (e.g., high accuracy ultrasonic flow instruments or temperature instruments). Include calibration records for the instruments used during these tests
2) A list of corrective action program documents (with a short description) associated with heat exchangers, heat sinks, silting, corrosion, fouling, or heat exchanger testing, that are documented in your corrective action system (for the previous 2 years or since the last biennial heat sink inspection), This request is for all the Generic Letter 89-13 HXs and the ultimate heat sink
3) Copy of system description and design basis document for the heat exchangers under review (as applicable)
4) Copy of any operability determinations or other documentation of degradation associated with the heat exchangers or the systems that support the operation of the heat exchangers
5) Copy of any self-assessment done within last 2 years on any of the licensees heat exchanger programs (e.g., Generic Letter 89-13 program heat exchangers) or on the sample heat exchangers
6) A schedule of all inspections, cleanings, maintenance, or testing of any plant heat exchanger to be done during the on-site portion of the inspection 1 Enclosure

7) Listing of the GL 89-13 heat exchangers by risk significance
8) List of operability evaluations currently relied upon and those that were previously (past 2 years) relied upon for operability
9) Copy of the evaluations of data for the two most recent completed tests confirming the thermal performance of each heat exchanger
10) Provide copy of the calculation which establishes the limiting (maximum) design basis heat load which is required to be removed by each of these heat exchangers
11) Copy of the calculation which correlates surveillance testing results from these heat exchangers with design basis heat removal capability (e.g., basis for surveillance test acceptance criteria)
12) The clean and inspection maintenance schedule for each heat exchanger for the next five years
13) Copy of the document describing the inspection results for the last two clean and inspection activities completed on each heat exchanger
14) Copy of the document which identifies the current number of tubes in service for each heat exchanger and the supporting calculation which establishes the maximum number of tubes which can be plugged in each heat exchanger
15) Copy of the document establishing the repair criteria (plugging limit) for degraded tubes which are identified in each heat exchanger
16) Copy of the design specification and heat exchanger data sheets for each heat exchanger.
17) Copy of the vendor manuals including component drawings for each heat exchanger.
18) Copy of the calculations or documents which evaluate the potential for water hammer or excessive tube vibration in the heat exchanger or associated piping.
19) Copy of heat exchanger performance trending data tracked for each heat exchanger.
20) Copies of those documents that describe the methods taken to control water chemistry in the heat exchangers.
21) Copies of the documents that verify the following for the ultimate heat sink:
a. Sufficient reservoir capacity. This includes:

maintenance records or methodology used including results of underwater diving inspections which demonstrated UHS capability, (e.g., confirmation of UHS 2 Enclosure

BIENNIAL HEAT SINK INSPECTION DOCUMENT REQUEST capacity by detecting and removing sediment intrusion as necessary, and confirmation of structural integrity of underwater UHS structures by inspection;

b. Functionality during adverse weather conditions, e.g., icing or high temperatures; and
c. Last two performance tests for all pumps and all valves in the safety-related service water system. If the components are not performance tested, please provide documentation verifying performance by the methods actually used.
22) Copies of the documents that verify flow testing was done at maximum design flow during the last 2 years, or since the last time it was done. Copies of the documents that verify maximum design flow testing will continue to be periodically done in the future.
23) Copies of the documents that verify the structural integrity of the heat exchanger, e.g.,

eddy current summary sheets, ultrasonic testing results, and visual inspection results.

24) Provide a list of calculations with a description which currently apply to each HX.
25) Provide the Design Basis Documents for the above listed HXs.
26) System health report(s) and maintenance rule system notebooks for these HX(s);

27) Copies of procedures developed to implement the recommendations of GL 89-13, Service Water System Problems Affecting Safety-Related Equipment, e.g., the GL 89-13 Heat Exchanger Program description.
28) List of engineering-related Operator Workarounds/Temporary Modifications for these HX(s) since the last Heat Sink Performance.
29) Copy of self-assessments and audits for the selected HXs and for the GL 89-13 Heat Exchanger Program.
30) Updated Final Safety Analysis Report pages for these HX(s) and for the GL 89-13 Heat Exchanger Program.
31) Information regarding any alarms which monitor on-line performance of these HXs.
