DR-03-05-026 MODIFICATION 5 PAGE 2 OF 4 The purposes of this modification are to increase the contract ceiling by $14,270 to allow for a third site visit to Entergy's Plant, Vermont Yankee, to resolve remaining open items and to provide incremental funds in the amount of $27,144 to fully fund the delivery order. The specific changes to the contract are identified below:

1. Attachment 1 -Statement of Work Task 3, Conduct and Document the AMRs Review Add the following sentence at the end of the first paragraph:

To address unresolved open items from the initial audits, the contractor shall participate in an additional audit trip to Vermont Yankee of up to three days for one person.

Delete the last sentence in its entirety and replace with the following:

For planning purposes, the draft audit and review report shall be delivered to the TM no later than ten (10) working days after the last on-site visit.

Section 5. Meetings and Travel Add the following trip under the subsection, "Travel":

2. Attachment 2 - Schedule of Services NRC - Summary of Palisades and Entergy Plant Audit and Review is hereby deleted in its entirety and replaced as follows to reflect the within scope change to the Entergy Plant:

4. The total ceiling price for Entergy Plant is increased by $14,270.00, from $246,603.00 to

$260,873.00. The total ceiling price of the order is increased by $14,270.00, from $550,141.00 to $564,411.00.
