(]nclude initiatives taken, lessons learned, effectiveness evaluation, etc. as appropriate.)
General Note:
Duke randomly tests at a rate of at least 50% of the pool population.

Beginning with the last semi-annual report (period ending 05/30/01), Duke will provide a more detailed report that will indicate positive test results confirmed at the NRC cut-off level and Duke's cut-off level. This detailed report for the period ending 06/30/06 is attached (see Attachment B).
Note (2):
Duke has included Random Pre-Access screen numbers in the total random numbers on the initial sheet. The number of Random Pre-Access has been broken out on the detailed report (see Attachment B).

Breakdown of Confirmed Positive Tests for Specific Substances - Duke General Office (for 6 Month Ending 06/30/06)
A
Attachment A Fitness for Duty Program Performance Data Personnel Subject to 10CFR 26 (for Duke General Office) for 6 Month Ending 06/30/06 Summary of Management Actions:
(Include initiatives taken, lessons learned, effectiveness evaluation, etc. as appropriate.)
General Note:
Duke randomly tests at a rate of at least 50% of the pool population.
Note (1):
Duke has more restrictive cut-off levels for Alcohol (due to range of extrapolation) and Marijuana. In the past, Duke has shown this detail on the Performance Data Report's first sheet. Beginning with the last semi-annual report (period ending 06/30/01), Duke will provide a more detailed report that will indicate positive test results confirmed at the NRC cut-off level and Duke's cut-off level. This detailed report for the period ending 06/30/06 is attached (see Attachment B).
Note (2):
Duke has included Random Pre-Access screen numbers in the total random numbers on the initial sheet. The number of Random Pre-Access has been broken out on the detailed report (see Attachment B).
Note (3):
Duke is reporting data from an approved contractor/vendor FFD program (Westinghouse) to do pre-access drug/alcohol screens only. Therefore, there is no data available for the "Average Number with Unescorted Access" field for the General Office Performance Data Report because Duke General Office does not have a vendor/contractor random pool; this data is reflected in the three nuclear site Performance Data Reports.
Note (4):
Duke approved the Pre-Access Testing portion of Westinghouse's Fitness for Duty Program effective August 3, 1999.
The 20 Pre-Access screens reported under "Short Term Contractor Personnel" are being reported with Duke performance data as a result of accepting Westinghouse's program.
