SUBJECT:

Nine Mile Point Nuclear Station Unit No. 1; Docket No. 50-220 Special Report: Channel #11 of the Containment Hydrogen Monitoring System Inoperable for Unplanned Maintenance In accordance with Action 4.a of Nine Mile Point Unit I Technical Specification Table 3.6.11-2, "Accident Monitoring Instrumentation Action Statements," Nine Mile Point Nuclear Station, LLC, is submitting the following Special Report concerning inoperability of Channel #11 of the Containment Hydrogen Monitoring (CHM) System.

Descriptionof Event On June 4, 2006, at 0021 hours2.430556e-4 days <br />0.00583 hours <br />3.472222e-5 weeks <br />7.9905e-6 months <br />, operators declared Channel #11 of the CHM System inoperable due to low sample flow.

Cause of Jnoperabilitv Channel #11 of the CHM System was declared inoperable due to low sample flow.

Troubleshooting determined that the cause of low sample flow was a malfunctioning return sample flow switch. Normal sample flows were not impacted and oxygen and hydrogen indications were available in the Control Room, therefore Channel #11 of the CHM System was able to perform its finction as designed.

Channel #12 of the CHM System was verified to be operable prior to removing Channel #11 from service.

Actions Taken The malfunctioning sample flow switch was replaced, post maintenance testing was performed satisfactorily and Channel # I1of the CHM System was restored to operable status on June 8, 2006, at 1316 hours0.0152 days <br />0.366 hours <br />0.00218 weeks <br />5.00738e-4 months <br />.
