1.0 INTRODUCTION

This report is submitted in accordance with Technical Specification 4.4.4.5.b. that requires the complete results of the steam generator tube inservice inspection from Refuel #13 to be submitted in a special report pursuant to Technical Specification 6.9.2, within 12 months following completion of the inspection.

The report includes:

Number and extent of tubes inspected. (Table 3.1)

2.

Location and percent of wall-thickness penetration for each indication of an imperfection. (Attachments 2 and 3)

3.

Identification of tubes plugged or sleeved. (Tables 5.2 and 5.3)

The steam generator tube inservice inspection was performed to meet and exceed the requirements of Technical Specification 3/4.4.4. Waterford 3 performed all S/G eddy current testing examinations in accordance with EPRI 'PWR Steam Generator Examination Guidelines" and its Appendix H qualified techniques.

2.0 Steam Generator Design The Waterford 3 Model 70 re-circulating steam generators were designed and fabricated by Nuclear Steam System Supplier Combustion Engineering (CE), Inc. in accordance with ASME Code,Section III NB for Class I vessels. The Waterford 3 steam generators each consist of 9,350 high temperature mill annealed Inconel 600 U-tubes arranged in a one-inch inner diameter triangular pitch pattern representing 103,574 ft2 of heat transfer area. The U-tubes are 3/4" O.D. by 0.048" nominal wall thickness explosively expanded the full depth of the tubesheet (CE's Explansion Process) and welded to the primary cladding. The secondary tube bundle support structure consists of carbon steel eggcrates. The secondary supports are arranged in the following order:

The Refuel #13 S/G eddy current testing (ECT) program consisted of the inspection plans listed in Table 3.1 The inspection included 100% of tubes Tube End Hot to Tube End Cold (bobbin) and 100% Hot Leg Top of the Tubesheet (rotating coil) +3 to -11 inches.

  • NOTE: The U-Bend Low Row (1-2) region of the respective tubes in each SIG were examined with "+Pt. "and straight sections Inspected with bobbin coil.

3.2 Tube Integrity Acceptable tube integrity was demonstrated at the end of Cycle 13 and condition monitoring requirements on burst pressure (3ANOP) and accident leakage rates were satisfied for all forms of degradation mechanisms identified during the Refuel #13 S/G eddy current testing. All indications identified were well below their associated structural limits. The Waterford 3 Refuel #13 Condition Monitoring Report was documented via Westinghouse Document SG-SGDA 23, 'Waterford 3 RF13 Steam Generator Condition Monitoring Report and Interim Cycle 14 Operational Assessment." The report is contained under Entergy's Document ER-W3-2005-0226-000.

As part of Waterford 3's adherence to perform In-Situ Screening in accordance with EPRI, TR-107620, "Steam Generator In-Situ Pressure Test Guidelines",

Rev. 1, all indications identified were screened with no tests required.

3.3 Degradation Mechanisms During RE 3, indications were detected that were suggestive of the following degradation mechanisms:

Axial ODSCC at non-dented hot and cold leg eggcrate intersections (SG31 and SG32)

Axial ODSCC in the hot leg sludge pile freespan region and expansion transition region (SG31 and SG32)

Circumferential ODSCC at the hot leg TTS expansion transition (SG31)

Circumferential PWSCC at the hot leg expansion transition and expanded tubesheet region (SG31 and SG32)

  • Axial PWSCC at the hot leg expansion transition and expanded tubesheet region (SG31)

Wear at eggcrate, diagonal bar, and vertical strap sites (SG31 and SG32)

Tube wear due to foreign objects (SG31 and SG32)

Attachment I to W3Fl-2006-0024 Page 4 of 9 A new degradation mechanism discovered during RF13 is 2 failed diagonal bars (Batwings) within SG #2. The failed batwings were identified through eddy current signal analysis and confirmed with visual examination. The condition is entered into the site's Corrective Action Program (CR-WF3-2005-01977). The response to the condition was discussed with the NRC Staff through several conference calls and site resident interface. The NRC Staff provided a letter dated March 31, 2006 (TAC NO. MC6047) to the site for close out of these discussions. A rigorous assessment of the condition was completed in December 2005 which modeled the thermo hydraulic conditions and wear. The assessment concluded that operation until the end of Cycle 14 was acceptable.

This Complicated Assessment was added to the existing Operational Assessment results.

NDE techniques used to detect the identified degradation mechanisms are included in Attachment 5 to this report. Attachment 4 provides a tabular summary of the tubes repaired due to various degradation mechanisms. The summary does not include tubes plugged as a response (Corrective Action) to the 2 failed diagonal bars (Batwings) under the site's Corrective Action Program.

4.0 Location and Percent of Wall-Thickness Penetration for Each Indication of an Imperfection The inspection data is summarized in Attachments 2 and 3. The only flaws sized and left in service are for wear. All wear > 40% through wall was removed from service by plugging.

5.0 Percentage of Tubes Plugged or Sleeved Table 5.1 list the repairs made in RF13 and the cumulative repairs to date:

N/A Preventative plugging and stabilization was completed to address potential concerns with the batwings under actions contained in the Corrective Action Program response. contains the repaired tubes by various degradation modes exclusive of the batwing corrective actions. The batwing response required additional plugging of 124 in SG #1 and 166 in SG #2. These tubes did not contain flaws.

to W3FI-2006-0024 Page 5 of 9 On May 11, 2005 Waterford 3 successfully completed all S/G eddy current examinations and plugging activities. Additionally, Waterford 3 performed visual inspection of the S/G tubesheets for evidence of leaking tube plugs. As a result of the tubesheet scans, Waterford 3 did not identify any leaking tube plugs, thus closing the S/G inspection. The SG bowl closeouts were completed on May 11, 2005 to complete the RF#1 3 inspection.

The SIG man-ways were re-installed on May 17, 2005.

Tables 5.2 and 5.3 include the list of tubes plugged during Waterford 3's RF13 Outage.
