OC Visitor Form Attached you will find a form that we are going to need to have filled out for each person going inside the Protected Area (inside the fence). This would be the people that would be visiting the intake structure area and the person walking down the rad monitors.

Along those lines, for the person(s) going to the rad monitor(s), there would be additional paperwork necessary. We would also like to know which rad monitors you would like to see since that will determine the paperwork that would be needed to walk down this Cat 1 item. RAGEMS, and other process monitors are inside the RCA. Monitor outputs may be in the Control Room which is a vital area.

If you could get back to me on this, I would appreciate it.
