Dear Mr. Levis:
On May 3, 2004, the Region I Field Office of the NRC Office of Investigations (OI) opened an investigation (Case No.1-2004-025) to determine if there was a deliberate violation of technical specification requirements related to the testing of Salem Unit 2 high head safety injection valves SJ-12 and SJ-13 on June 21, 2001.
OI identified that initial results from a surveillance test performed to determine the valve leakage indicated that the leakage from these valves was above the Technical Specification Limiting Condition for Operation for reactor coolant system leakage. However, the valves and piping in the area were hot and the measured flow was not constant, indicating that water through the valves was flashing to steam. Since the method of measurement of valve leakage assumes water flow, these surveillance test results were neither accurate nor valid. The temperature condition was corrected and the surveillance was performed under proper conditions. The subsequent surveillance test results showed that the valve leakage was in compliance with Technical Specification requirements. No violation of NRC requirements was identified.
