USA Assessment Preliminary Results On Friday (March 5), I spoke w/ Jim Hutton (HC Plant Manager) about the preliminary results of USA Assessment that was recently completed at the site. The USA Assessment was performed during the week of March 1, and the team conducted a debrief on March 5 and presented the results to PSEG managers and supervisors (including F. Cassidy). A written report will be provided to PSEG within 30 days of the exit meeting.
J. Hutton shared the following results with me:
Overall, the assessment did not identify any new issues. However, it did confirm a lot of the issues/themes that the Synergy survey identified.
- 1. Material condition of plant is poor (ex Service Water, EDG, CRDMs)
- 2. Corrective Action process has not been fully effective.
- 3. Work Management process lacks ownership
- 4. PSEG personnel have noticed an increased management involvement in the plant (I believe that this observation is based on the current management team's actions)
In addition, the Assessment team commented on HC's reactivity management performance. In particular they commented on the following events: 1) March 19, 2003 - Stuck Open Turbine Bypass Valve; and 2)
Sept 29, 2003 - Rx taken critical - subcritical-critical during RCIC warming.
