I The NRC has received information regarding the performance of certain activities at Salem and Hope Creek that may be not meet acceptable station standards, procedures and practices. The concerns are listed below.
Statement of Concerns:
- 1.
First line supervision does not effectively direct workers to follow correct procedures for dressing out after entering contaminated areas. A specific example involves an incident where painters were allowed to walk through clean areas between the Salem RHR rooms without fully removing the appropriate protective clothing.
- 2.
Sandblast material was observed falling on the Salem containment spray pumps during a painting activity, calling into question the adequacy of FME controls. The operability of the containment spray pumps has called into question whether sandblast-material may have penetrated the internal areas of the motor.
- 3.
Allegedly, after graffiti was discovered in the Hope Creek torus, a second layer of paint was applied to cover the graffiti without adequate surface preparation. If, over time, the paint layer were to chip off this would constitute a FME concern.
- 4.
It is not readily apparent whether the following conditions were adequately documented in PSEG's notification system or effectively addressed: missing or loose ground straps on pipes in the service water intake structure; and, rust on a ground strap on one of the chillers that was painted over.
