MID-CYCLE PERFORMANCE REVIEW AND INSPECTION PLAN - LASALLE COUNTY STATION
Dear Mr. Crane:
On August 9, 2005, the NRC staff completed its performance review of LaSalle County Station for the first half of the calendar year 2005 assessment cycle. Our technical staff reviewed performance indicators (PIs) for the most recent quarter and inspection results over the previous twelve months. The purpose of this letter is to inform you of our assessment of your safety performance during this period and our plans for future inspections at your facility.
This performance review and enclosed inspection plan do not include physical protection information. A separate letter designated and marked as Exempt from Public Disclosure in Accordance with 10 CFR 2.390 will include the physical protection review and resultant inspection plan.
Plant performance for the most recent quarter was within the Licensee Response Column of the NRCs Action Matrix, based on all inspection findings being classified as having very low safety significance (Green) and all PIs indicating performance at a level requiring no additional NRC oversight (Green). Although plant performance was within the Licensee Response Column, on May 12, 2005, frequently asked question (FAQ) 27.3 was denied by the NRC for the Scrams with Loss of Normal Heat Removal performance indicator. The denial changed the Unit 2 performance indicator from Green to White for the third and fourth quarter of 2003 and the first quarter of 2004. The white performance indicator requires that a supplemental inspection (95001) be performed. We have scheduled the 95001 supplemental inspection for late September, 2005.
The Mid-Cycle and End-of-Cycle assessments for 2004 discussed a human performance substantive cross-cutting issue that was identified by assessing plant performance utilizing NRC inspection procedure (IP) 71152 to review problem identification and resolution (PI&R) issues during routine baseline inspections and during the biennial PI&R inspection. Over the course
C. Crane of the assessment period, six Green findings were identified where human performance was not adequate. These findings were all personnel related and primarily associated with procedure use/procedure adherence issues. Four of these findings were in the reactor safety strategic performance area (SPA) (three initiating events and one mitigating systems cornerstone) and two in the radiation safety SPA (occupational radiation safety cornerstone).
To assess human performance corrective actions, our inspectors completed an inspection that focused on the implementation of corrective actions detailed in your human performance improvement plan. The inspection identified a reduction over the last three months regarding the frequency and significance of human performance errors during normal power operations.
However, a significant aspect of our review was to identify improvements in human performance during outages. The inspection was inconclusive regarding the frequency and number of human performance errors that had occurred during the recent Unit 2 outage.
Based on the inconclusive Unit 2 outage results, we plan to retain the human performance substantive cross-cutting issue open. We will reassess the cross-cutting issue after completion of the scheduled Unit 1 refueling outage in March 2006. We are also aware that additional enhancements were made to your human performance improvement plan to address the human performance errors that occurred during the Unit 2 outage. Therefore, we are requesting that you provide a letter to the NRC, within the next 90 days, detailing the specific enhancements you have made to improve human performance during outages at LaSalle County Station.
The enclosed inspection plan details the inspections, less those related to physical protection scheduled through March 31, 2007. Significant inspections planned include the 95001 supplemental inspection in September 2005, and a safety system design and performance capability/safety system modifications inspection in October 2006. We also plan to perform a focused inspection of human performance using Inspection Procedure 71152, Identification and Resolution of Problems. This inspection will select ROP findings with human performance issues for a more in-depth review. The inspection plan was provided to minimize the resource impact on your staff and to allow for scheduling conflicts and personnel availability to be resolved in advance of inspector arrival onsite. Routine resident inspections are not listed due to their ongoing and continuous nature. The inspections in the last nine months of the inspection plan are tentative and may be revised at the end-of-cycle review meeting.
In accordance with 10 CFR 2.390 of the NRCs Rules of Practice, a copy of this letter and its enclosure will be made available electronically for public inspection in the NRC Public Document Room or from the Publicly Available Records (PARS) component of NRC's document system (ADAMS).
