11 MR. DODSON: I am the Station Licensee 12 Supervisor. We are in an alert here at the station.

13 You may or may not be aware of that?

14 MR. GOTT: Yes, sir, we are aware.

15 MR. DODSON: Alright. I have an NRC Event 16 Notification form to cover the reporting details that 17 are necessary under 5072.

18 MR. GOTT: So this is a follow-up to the 19 other one?

20 MR. GOTT: Which one did you receive?

21 MR. DODSON: Hold on a second. Well, we -

22 - hold on a second. We received a call in for the 23 alert from Millstone Unit 3.

4 MR. GOTT: Let me see - let me call up the 5 event.

6 MR. DODSON: Okay.

7 MR. GOTT: I have been running these other 8 things here. Let me see. 41607.

9 MR. DODSON: That is correct, yes.

10 MR. GOTT: So this is an update to 41607?

11 MR. DODSON: Yes, it is.

12 MR. GOTT: Okay. You are calling in at 13 1328. Could I get your name again, sir?

14 MR. DODSON: My name is David Dodson.

15 MR. GOTT: David Dodson.

16 MR. DODSON: D-O-D, as in "David," S-0-N.

17 MR. GOTT: Okay, sir. And ybur update is 18 what?

19 MR. DODSON: Hang on one second. We are 20 reporting, under the Event Classification, an alert, 21 also a 5072 Non-Emergency Report.

22 MR. GOTT: Okay.

23 MR. DODSON: Under the column "4-hour Non-24 Emergency Events," we have checked "ECCS discharge 25 into the RCS."

2 MR. DODSON: That is correct. And also, 3 under RPS actuation.

4 MR. GOTT: And RPS actuation.

5 MR. DODSON: That is correct.

6 MR. GOTT: Okay.

7 MR. DODSON: And I could read you a 8 summary of the event.

9 MR. SANDIN: If you could, please, sir.

10 MR. GOTT: "At approximately 0830 on April 11 17, 2005, Millstone Unit 3 experienced an automatic 12 reactor trip from full power. The reactor is in a 13 stable shut-down condition and is proceeding to Mode 5 14 Cold Shut-Down. No release of radioactivity, other 15 than those minor releases associated with normal plant 16 operation has occurred. There were no personnel 17 injuries associated with the event. In addition to 18 the reactor trip, half-train safety injection had 19 half-train main steam isolation actuations occurred.

20 The Control Room personnel were successful at manually 21 initiating full SI and MSI." That is the safety 22 injection and main steam isolation signals. "The 23 Safety injection has since been terminated and RCS 24 pressure control has been restored. Multiple steam 25 line safety valves lifted, with at least one main NEAL R. 42 1 steam line safety valve indicating a failure to 2 reclose. As a result, an alert was declared at 3 approximately 0853, due to the failure of at least one 4 main steam safety valve to close. All main steam 5 safety valves are currently closed. Auxiliary feed 6 were actuated automatically as expected, however, the 7 turbine-driven auxiliary feed water pump tripped on 8 start-up and was subsequently reset locally. Both 9 motor-driven auxiliary feed water pumps operated as 10 expected to maintain steam generator water levels.

11 Due to the additional inventory injected into the 12 reactor coolant system as a result of the safety 13 injection actuation, the pressurizer filled and a 14 primary system safety valve actuated at a lower than 15 expected pressure. All primary safety valves are 16 currently closed. However, this is indication of 17 leakage past either a primary safety valve or a pilot is operated relief valve. Upon alignment of the charging 19 system to the refueling water storage tank re-20 circulation flow path, two valves had indication of 21 packing leakage. Those packing leaks have been 22 terminated. The cause of the event is under 23 investigation."

3 MR. DODSON: I have not spoken with the 4 resident. The Resident is in the Control Room at this 5 point in time and is aware of these details, though.

6 Okay.

7 MR. GOTT: Are you also reporting this as a 8 specified system actuation? The 8-hour B(3)4 Alpha?

9 MR. DODSON: You know, I did not look at 10 that, but yes, that would be applicable as well.

11 MR. GOTT: That is. I will mark that on 12 my sheet.

13 MR. DODSON: Yes.

14 MR. GOTT: Okay. And for the RPS 15 actuation, all control rods went --

16 MR. DODSON: Fully inserted.

17 MR. GOTT: -- fully inserted. And they are 18 removing the decay heat (phonetic) now through the 19 condenser?

20 MR. DODSON: I do not have that 21 information.

22 MR. GOTT: Okay. Alright then.

23 MR. DODSON: I believe that is the case, 24 but I do not have confirmation of that.

9 MR. DODSON: That is correct, to the best 10 of my understanding. Okay.

20 MR. DODSON: And it is what we call 21 "Attachment 8" which is our NRC Event Notification 22 form. That has two sheets and there will - the third 23 sheet will be the attachment that I just read to you.

24 MR. GOTT (phonetic): Okay. And you have 25 our fax number?

16 MR. GOTT: Okay. If it is not too hard, 17 could you fax a copy of those up to here also?

18 MR. DODSON: We can include a copy of 19 those documents in this fax as well.
