3 MR. LANNING: Okay. Bill, as Randy 4 indicated earlier, we'll continue to ask for a 5 response from the Region. With the termination of the 6 alert, does that mean I'll have a communicator and who 7 will that be and how will I know?

8 MR. HOFFNER: What we're going to do is

( e .- Pe) 9 even though we are going to terminate, by the time we 10 send out the termination, get the paperwork done and 11 do a debrief, we'll still have personnel here in EOF.

12 By the time we finish that, we anticipate we'll be in 13 mode 4 and we'll maintain communications with you 14 until that point.

15 MR. LANNING: All right.

16 MR. HOFFNER: I will stay available to 17 continue the communication with you until we're in 18 mode 4 regardless.

19 MR. LANNING: And mode 4 is what?

20 MR. HOFFNER: Less than 350 degrees.

3 1 let me canvas the group. Bill, have you any new 2 intelligence on the leaking PORV, which one, and so 3 forth?

4 MR. HOFFNER: No. We're going through our 5 alarm response to isolate the different PORVs. We've 6 isolated the alpha. As of right now that is not 7 indicative of the leak. Then we'll go over and do the 8 bravo very shortly. If not, then that would be 9 indicative of a potential leak in our safety valve.

10 There are some other inputs that go into that PRT but 11 we're going to be looking at that using other 12 indications.

13 MR. LANNING: I also understand that 14 you're not going to be resetting the main steam 15 isolation signal. Is that your understanding?

16 MR. HOFFNER: Could you say that again?

17 MR. LANNING: It's my understanding that 18 the operators will not be resetting the main steam 19 isolation signal.

20 MR. HOFFNER: When we began to cool down 21 the pressurization we blocked the steam isolation 22 signal and when that happened the signal reset. Are 23 you guys still there?

2 MR. HOFFNER: We're not going to at this 3 point reopen the valves right now. The signal, as far 4 as I'm aware, when we blocked the steam line low 5 pressure, when we blocked that by procedure, the alarm 6 cleared.

7 MR. LANNING: So I understand when you 8 blocked the steam line low pressure --

9 MR. HOFFNER: It's a permissive.

10 MR. LANNING: Yes.

11 MR. HOFFNER: And when RCS pressure got 12 below P11, which is our set point, it says block it.

13 When we blocked it, the steam line pressure low signal 14 went away.

15 MR. LANNING: Okay.

16 MR. HOFFNER: That was what was giving the 17 main steam line. Now, we have not reset the main 18 steam line isolation signal which is a manual signal.

19 MR. LANNING: Okay. That was what I was 20 asking about.

21 MR. HOFFNER: Right. The operators have 22 not depressed that push button.

23 MR. LANNING: Okay. And what is your 24 rationale there?

5 1 a review team investigation and we didn't want to 2 remove potentially some information as to the cause.

3 We don't need them right now to continue our cool 4 down.

5 MR. LANNING: Okay. I guess I was 6 thinking that you brought down some steam generators 7 and the atmosphere' release.

8 MR. HOFFNER: Well, the blow down doesn't 9 get a main steam isolation signal. That gets an aux 10 feed water isolation so we could reset that if we 11 wanted to. It's just some other procedures to get the 12 sample.

13 MR. LANNING: Okay. All right. Any other 14 questions?

15 MR. PARTICIPANT: This is headquarters 16 operation. I need to confirm that the operation 17 center here will get a separate call on securing from 18 the alert.

19 MR. HOFFNER: We will release an 20 notification and follow the communication procedures 21 once they actually do terminate.

22 MR. PARTICIPANT: Thank you, sir.

23 MR. HOFFNER: Okay.

6 1 MR. PARTICIPANT: This is the Operations 2 Center. When we exit from the alert, are we going to 3 entertain a conference for securing the agency in 4 monitoring?

5 MR. PARTICIPANT: We have generated some 6 criteria here for exiting the monitoring mode and it 7 goes a little beyond the termination of the alert 8 status. It will be sometime after they terminate the 9 alert.

10 MR. PARTICIPANT: Okay. Understood.

11 Thank you, sir.

12 MR. SCHNEIDER: Wayne in Region I, this is 13 Max.

14 MR. LANNING: Go ahead, Max.

15 MR. SCHNEIDER: I think Bill Hoffner 16 eventually got to the MSI reset perspective that we 17 passed on. Also, they had secured the procedure for 18 draining the PRT once they saw the new "leakage" 19 coming in. They have been around 68 percent or so for 20 some time now.

21 They also had a problem with their EEQ 22 data logger. I guess this brings in a lot of 23 temperature indications, etc. There might be some 24 complication there to evaluating downstream tail piece 25 temperatures between alpha and bravo PORV. 2 MR. LANNING: Okay, Max. Do they have 3 acoustic monitors at this point or just all RCDs and 4 temperature elements?

5 MR. SCHNEIDER: That's a good question.

6 I didn't check to see that. Let me see if there is 7 anything on the acoustic monitors.

8 MR. LANNING: Okay. Thanks. Anything 9 else? I think I'll get some PRT and another round of 10 plant indicators. That's all, Max.

11 MR. SCHNEIDER: All righty. Going off 12 line.

13 MR. PARTICIPANT: This is Silas at TOC.

14 I have an update on the RCP high range leakage flow 15 alarm.

16 MR. LANNING: Go ahead, Silas 17 MR. PARTICIPANT: Is someone talking on 18 line right now?

19 MR. LANNING: Yes, Silas. We have two 20 conversations going on. Go ahead and start your 21 debrief.

22 MR. PARTICIPANT: Okay. The alarm came in 23 as they were securing two of the four RCPs. They were 24 shutting down the Charlie and delta RCPs. They got 25 the alarm for the charlie RCP. This alarm, what it NEAL R. GROSS COURT REPORTERS AND TRANSCRIBERS 1323 RHODE ISLAND AVE., N.W.

8 1 is, it comes in if the seal flow is less than 1 gallon 2 per minute. So while the pump is running, the seal 3 flow should be one gallon per minute or greater. When 4 they secured the pump, the charlie pump seal flow went 5 to .8 gallons per minute and the alarm came in.

6 As the pump closed down the seal flow came 7 back up above 1 and the alarm went away. The purpose 8 of the flow of 1 gallon per minute is to ensure that 9 there is some lubrication between the rotating and 10 stationary elements of that seal.

11 There are three seals, primary, backup, 12 and vapor seal. The alarm was on the primary seal.

13 This was unexpected. It was an unexpected alarm but 14 because they did not get below -- it went to .8 15 gallons per minute, they don't believe there is a 16 problem with that particular seal.

17 MR. LANNING: Okay. I got it, Silas.

18 MR. PARTICIPANT: Okay.

19 MR. LANNING: Here's one follow-up. They 20 have dispositioned that alarm, though. They 21 understand it and --

22 MR. PARTICIPANT: We'll call you back here 23 as soon as they do.

24 MR. LANNING: It's not a complicating 25 event. Is that right?

9 1 MR. PARTICIPANT: That's right. It's not 2 a complicating event. It was unexpected but they 3 don't believe there are any problems with the seal.

4 MR. LANNING: Okay. Thank you, Silas.

5 MR. PARTICIPANT: Okay.

6 MR. SCHNEIDER: Wayne, Randy or Paul, this 7 is Max in the unit 3 control room.

8 MR. LANNING: Go ahead, Max.

9 MR. SCHNEIDER: Approximately 5:38 update.

10 Steam generator water level in all four is about 76 11 percent. Pressures are about 320 pounds (steam 12 generator pressure) . RCS temperature is about 430 13 degrees. Pressurizer level is about 71 percent. Cool 14 down rate continues at about 60 degrees per hour.

15 RCS pressure is about 730 pounds.

16 Containment pressure remains about 13.8 pounds. PRT 17 parameters, temperature is about 185 degrees. Level 18 is consistent fairly at 68 percent which is was 19 reported about 4:30. Pressure is about 42 pounds.

20 That's gone up a little bit.

21 Reactor coolant pumps alpha and bravo are 22 running, so charlie and delta are secured and the PORV 23 valve condition is that the alpha PORV is still closed 24 so they have yet to continue in the troubleshooting 25 procedure to get to a point to close bravo yet. 10 1 MSI signal as previously reported removed but not 2 reset. That's what I have.

3 MR. LANNING: Okay, Max. Thanks for that 4 update. Has the control room been notified or 5 declared that they are out of the alert yet?

6 MR. SCHNEIDER: I have not heard that.

7 MR. LANNING: Okay. Just checking.

8 MR. SCHNEIDER: Yes.

9 MR. LANNING: Thank you.

10 MR. PARTICIPANT: This is Headquarters 11 Operations Officer again. I've got Bill Hoffner 12 again, wants to be added to the bridge.

13 MR. LANNING: Okay.

14 MR. HOFFNER: Bill Hoffner on line.

15 MR. LANNING: Hi, Bill. Wayne Lanning.

16 MR. HOFFNER: Hey, Wayne. When we got 17 back together at 1730 we noticed the steam pressure 18 came down. The cool down rate changed. We made a 19 decision here that we will stay activated until we hit 20- mode 4 also.

21 MR. LANNING: All right. I understand 22 you're going to stay in the alert until you reach mode 23 4 which is 350 degrees RCS temperature?

11 1 down rate so we're still looking prior to 1900 being 2 there. We're going to stay activated until that 3 point.

4 MR. LANNING: What was the first part of 5 your statement about the steam pressure?

6 MR. HOFFNER: Just as you bleed steam 7 secondary pressure drops off and you have to open the 8 valves further. We didn't like what we saw for a 9 little bit in the rate so we decided not to terminate.

10 Since we're this close anyway, we're just going to 11 stay activated until that point.

12 MR. LANNING: Help me understand what you 13 mean by that. What are you telling me?

14 MR. HOFFNER: We use the throttle valves 15 so as steam pressure drops behind them, you have to 16 open them further to get the same cool down rate.

17 MR. LANNING: Right.

18 MR. HOFFNER: So we just saw that the cool 19 down rate was dropping off and we just got a little 20 concerned that we were going to be able to maintain 21 that rate and we decided not to terminate at 1730. By 22 adjusting the valves further open we have now 23 reestablished the 60 degree per hour cool down rate so 24 we are comfortable we're going to make it but since 25 we're so close anyway, we're just going to stay at NEAL R. 12 1 mode 4 to be concurrent with what you're doing.

2 MR. LANNING: Okay. Great. Appreciate 3 that.

4 MR. HOFFNER: Okay. Also, additionally --

5 okay. We're still going through the process of 6 isolating the PORVs. We are going to evaluate whether 7 we were successful in that. We are also in the 8 process of placing blow down on the delta steam 9 generator in service right now so we should have a 10 sample once we get that going.

11 MR. LANNING: Okay. I understand you're 12 reestablishing steam generator D blow down.

13 MR. HOFFNER: Delta. Correct. Okay, 14 Wayne?

15 MR. LANNING: Yes, sir. We're canvasing 16 as a group here to see if there are any other 17 questions.

18 MR. PARTICIPANT: Just the one on that 19 throttle valve that you were adjusting the cool down 20 rate. Are those locally controlled or was that the 21 controller from the control room, or remote?

22 MR. HOFFNER: No, those are motor operated 23 valves that are throttled. They are controlled from 24 the main control board.

13 1 any problem with the controller. You were trying to 2 adjust the valve based on lowering steam pressure.

3 MR. HOFFNER: Correct. And just based on 4 our concern for a little bit, we decided not to 5 terminate and right now we have reestablished the cool 6 down rate that we desire. Due to the fact we are 7 under an hour we are just going to wait to get to mode 8 4 at this point.

9 MR. PARTICIPANT: Okay. Got it. All 10 right.

11 MR. HOFFNER: My intention still would be 12 to call you just prior to the termination.

13 MR. PARTICIPANT: I would appreciate it, 14 yes.

15 MR. HOFFNER: Okay.

16 MR. PARTICIPANT: Thank you.

17 MR. HOFFNER: Okay. Any further questions 18 for me?

19 MR. PARTICIPANT: Max, do you have any?

5 MR. SCHNEIDER: This is Max in the unit 3 6 control room back on the line after observing an 7 internal brief.

8 MR. PARTICIPANT: Max, this is Silas. Are 9 you on?

10 MR. SCHNEIDER: Yes, I am.

11 MR. PARTICIPANT: Have you heard anymore 12 about many safeties lifted? All I know is in the 13 control room log it talks about two liftings, but I 14 haven't got any confirmation on what they believe.

15 MR. SCHNEIDER: They are just briefing the 16 PEOs oncoming. They remarked on the bravo steam 17 generator safety lifting 40 pounds below its set point 18 and then others kind of more probably characterized as 19 simmering or whatever, but that's the one that they 20 mentioned. That will be part of our follow-up review 21 to determine whatever special inspections team or 22 whatever to determine the exact circumstances and a 23 further detailed look at the data.

15 MR. SCHNEIDER: Yes.

16 MR. PARTICIPANT: The tampering procedure 17 was completed at 1:05 p.m. No evidence of tampering 18 or sabotage. Do you want a copy of the results of 19 that today or would tomorrow be better?

20 MR. SCHNEIDER: We're going to be heading 21 back to the office anyway by the time we're done.

22 MR. PARTICIPANT: Right.

23 MR. SCHNEIDER: Bring us a copy and put it 24 in our safe.

4 MR. SCHNEIDER: Silas, let me go over what 5 I thought I heard. You're saying that they completed 6 their tampering procedure at 1:05 this afternoon?

7 MR. PARTICIPANT: That's correct, yes. No 8 evidence of tampering or sabotage.

9 MR. SCHNEIDER: Okay. Thank you.

10 MR. PARTICIPANT: That we really did 11 implement it or --

12 MR. SCHNEIDER: I want to see the whole 13 thing.

14 MR. PARTICIPANT: Right.

15 MR. SCHNEIDER: Wayne, Randy, Paul, Region 16 I, this is Max.

17 MR. LANNING: Hey, Max. You have us all 18 here. Go ahead.

19 MR. SCHNEIDER: I just thought I would do 20 kind of like an hourly update with you guys and gals.

21 Steam generator water level 78 percent. Steam 22 generator pressure 160 pounds. RCS temperature is 370 23 degrees. They are still maintaining about 60 degrees 24 per hour cool down rate.

25 Pressurizer level still around 70 percent.

17 1 RCS pressure is about 440 pounds. Containment 2 pressure remains at 13.8. The PRT temperature is 185, 3 level 68 percent. The pressure is about 30 pounds.

4 Reactor coolant pump A and B are running.

5 PORV update. The bravo PORV was isolated 6 as part of the ARP troubleshooting. When they 7 isolated that the downstream common tail piece 8 temperature decreased about 20 degrees in 25 minutes.

9 Therefore, they are considering the bravo PORV to be 10 the leaker and the bravo PORV will remain isolated.

11 Steam generator, chemistry yet to be 12 determined. They have yet to finished lining up so 13 according to the steam generator chemist supervisor he 14 thinks probably an hour after they get them lined up 15 to get all the samples with delta and alpha being the 16 first and the second priority. That's it.

17 MR. LANNING: Max, did we ever ascertain 18 whether both PORVs opened or just one?

19 MR. SCHNEIDER: No, we didn't. The report 20 from the operator was the PORVs opened. He thought 21 the PORVs opened first so the thought there was that 22 probably the safeties didn't. It was probably just 23 the PORVs but he didn't know which one per se might be 24 the leaker. Now they've isolated it to bravo.

1 sequence of events?

2 MR. SCHNEIDER: Absolutely. At some point 3 they are going to have to put all the data together 4 and figure out what things didn't work and at what 5 time.

6 MR. LANNING: So we're an hour and a half 7 away from the delta generator sample. They've got to 8 finish the line up and then take a sample.

9 MR. SCHNEIDER: Right. That's probably a 10 pretty good estimate, Paul, because they are in the 11 midst of turnover as well so that's what happens here.

12 They are probably getting ready to go do something and 13 any of us who have stood watch as part of a crew of 14 some nature recognize that turnover often stops.

15 Where you stop doesn't get started up again for 16 another couple hours.

17 MR. LANNING: Max, we have another hour 18 and a half until 2029 time, tech spec 7.3.1, I guess 19 it is. They were initially estimating 1730, 5:30. As 20 I understand it, the LCO is going to expire 2029.

21 MR. SCHNEIDER: Correct. Given 12 hours1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br /> 22 the entry this morning was 8:29 so that would be 8:29 23 tonight. 19 1 MR. LANNING: Yeah, but, Max, they're not 2 near 60 degrees an hour.

3 MR. SCHNEIDER: Excuse me?

4 MR. LANNING: I don't think their cool 5 down rate is even approaching 60 degrees.

6 MR. SCHNEIDER: The ERDS data we're getting 7 shows about 45, maybe 50 max.

8 MR. LANNING: You might want to point that 9 out to somebody, Max.

10 MR. SCHNEIDER: Unless our ERDS data's way 11 out of wack, but our ERDS data's been right all day.

12 MR. LANNING: Absolutely. Even at 50 13 degrees an hour or 40 degrees an hour, they only need 14 to go 20 more. Right? I mean, more than 20. I mean, 15 they are going to get there before 8:29 unless they 16 hit a wall of some nature.

17 MR. SCHNEIDER: They have 20 more degrees.

18 We'll make it with 10 or 15 minutes to spare but that 19 will be about it.

20 MR. LANNING: Okay. I'm convinced that if 21 nothing happens they are going to make that in 30 22 minutes or so. Forget that alert.

22 1 that they will probably de-man the EOF as soon as they 2 come out of the alert but sometimes they keep the TSC 3 with a minimum staffing. That's what they did during 4 the security event for longer than that. They could 5 have the TSC staff until tomorrow.

6 MR. KROHN: Well, we'll see. We'll just 7 go with the flow.

8 MR. PARTICIPANT: Max, are you still 9 planning on turning over with Pete when he walks 10 through the door? Is that what you plan?

11 MR. SCHNEIDER: Yeah. He'll come up to 12 the control room. I'll do a turnover with him. Then 13 I'll go down and after I get turned over with Pete 14 I'll get Silas and then we're going to exit the 15 station at that point.

Then we'll go 23 from there, Silas and I, on how to respond to any 24 further in terms of this type of thing. Then Kevin 25 will go home.
