RED COMMUNICATION TRANSIENT COMBUSTIBLES IN CONTAINMENT Effective immediately, all combustibles taken into containment require a "Transient Combustible Permit" unless they are under your direct control at all times.

Examples of exempt items:

Work Package and associated paperwork Clip boards RED communications require completion of a QF-1060-02 'DOCUMENTATION OF INFORMATION SHARING" Completed briefing forms are to be brought to the OCC.

Don't pile up tools and equipment In the walkway even for short periods of time. Always keep the egress paths open for use.

Monday

Do you know the proper location to go In the event of a tornado warning if you are in the trailers, turbine hall or PAS? NP1.9.21 covers the site's various shelter areas. Plan ahead and take the time now to review the NP with your group.

Tuesday

OE15310 Indian Point3-An oil vaporextractorforthe main boilerfeedpump failed. The tag out used to repair the extractor resulted In an Increased leakage of oil from the pump bearings onto lagging. A special log was created to monitor the leakage, but no plan existed for dealing with the safety hazards.

Maintenance repaired the extractor, returning It to service. The next day a portion of oil soaked lagging on a main boiler feed pump caught fire and was quickly extinguished by an alert watchstander.

Last week on the 66ft of containment, the NRC noticed we blocked access to 2 fire extinguishers mounted on the stairwell handrail. A scaffold was built in the area that made access questionable. We then added a number of bags of Insulation It made access Impossible. Just a question - Why Is the NRC finding this Instead of us? .

Thursday "What number do you call if your co-worker takes a serious fall?"

What Is the plant's emergency number? 2911. This number Is only to be used for emergency situations.

Two weeks ago a work group was having trouble getting through to operations to communicate a change in equipment status. Rather than waiting, the Individual Involved decided to contact the control room via the emergency number. Needless to say this was not an appropriate use of 2911.

Friday

'Make sure your fire watch is alert to keep someone from getting hurt" OE12607 - LaSalle - A worker using an acetylene torch for cutting through a structural member that was behind a metal wall panel. Wall Insulation behind the member began to burn. The alert fire watch extinguished the fire within one minute of it beginning and no significant damage occurred.

Saturday "Debris buildup may result in an unplanned heat up" OE14899 River Bend Station -A small fire occurred under the exhaust piping on a diesel generator during testing. The fire was observed during routine checks of the generator and the control room was Immediately notified. The control room unloaded the generator while the operator extinguished the fire. The cause was Insulation debris that accumulated around the hot piping over time.

This indicator measures cumulative dose radiation exposure and Day 37 - May 10 Actual = 0.257 total number of personnel-contamination events (PCE's > 5000 Cumulative = 54.171 cpm) during refueling outages. The dose indicator is measured in Cumulative Forecast = 54.403 Rem and individual PCE events.

Daily PCE goal reforecast on 4/23. Daily dose reforecast on 5/8/04.

Meets: <=75 Rem Actual Cum. Reactor Head Repair Dose for May 10th Is 1.799 vs. goal of .540. RV Head Exceeds: <=71 Rem Dose: 54.171 Rem Total Dose Is 3.028.

Meets: <=18 Exceeds: <= 12 Actual PCE's: 9
