14.256 Rem Received more dose than expected for a valve packing lob in the regen Hx area.

PCE #2 recorded 4M7D04 - individual was removing scaffolding In Unit 1 Containment Keyway. Found to have 8,000 cpm contamination on his hand plus lower doses on his arm, modesty top and bottom. Actions were taken to remove the contamination. CAP written.

TEAM REPORT CARD WEEK B02BI, 3128-41312004 Work Week Coordinator: Rob Geier Analysis:

U1R28 refueling outage started on Friday evening of this workweek.

Emergent work U2 was ramped down to 83%, due to SI Accumulator level high CAP055217 & 055204. An extra U2 TS-ECCS-002 venting & 3 more calibrations of SI Accumulator level transmitters were required. 2LT-938 out of tolerance CAP055230. Many unplanned U2 containment entries were required.

  • Prepared for 2CV-285 Freeze Seal & repair, but did not execute the SMP.
  • Temporary power to XL-1 0. Lost more equipment than expected (SW header Zurn delta-P lost CAP055294) (Ul Fagade elevator lost CAP055295)
  • DY-OD; replaced 5 cards and was able to return it to service
  • 2SI-825B MOV failed to open completely during U2 IT-02 CAP055226 W-30B motor breaker tripped on 3/27/04 CAP055161. Removed W-30B motor for vendor repair. MCE completed last November show this motor was ready to fail, but WO was not written, CAP055200.

Restraints to Scheduled Work:

Expanded scope: Pre fab of 1 SI-845A failed it's leak test CAP055305 Package Readiness: SW-4438 spare RV is missing, Pre Outage rebuild of 1SI-817A-O delayed due to incorrect drawing CAP055216 Resources: ICP 6.1 on BA Heat trace recorders, Misc Electrical work for TG-01 & CE insulation removal for orifices did not complete due to resources ENG/Construction: The installation of WT pretreatment did not complete due to a design error.

This also affected OPS baseline activity to remove tags.

Plant Conditions: Divers were not able to install buoys in the lake due wind & waves. Scaffolding &

Insulation removal for 1 MS-1 17 was delayed until U1 shutdown due to a steam leak in the area.

SI/RHR testing on both units was executed smoothly and as scheduled.

On Monday, 3/29/04, EM craft asked the WCC if they could swap the breaker for SW-32F motor.

This was not in the schedule. The breaker swap was planned on Friday 3/26/04 ion B01, but due to problems with SW-2912-BS & SW TSAC, it could not be work on Friday. EM Supervision was notified on Friday that this would be rescheduled until after the outage and could not be worked in

  • B02. CAP055183 9 work orders were not completed prior to the start of U1R28 as planned and were added to the outage.
