During the night shift of April 8 to the morning of April 9, a near miss occurred having no Unit I hot leg vent path for a period time during the nozzle dam installation. During the night of April

8. the removal of the pressurizer manway experienced a problem with removal of all of the bolting. The OCC performed an evaluation of the schedule to determine the ability to perform activities in parallel. The scheduled required that the pressurizer man way be removed to provide a hot leg vent path prior to installation of either RCS hot leg nozzle dam in order to ensure compliance to GL 88-17 and NUREG-1449.

A near miss occurred when workers proceeded to install the hot leg nozzle dams prior to the pressurizer man way being removed. The workers had positioned the nozzle dam in the SIG bowl, but had not fastened it in place when they were instructed to remove the nozzle dam from the SIG bowl until a hot leg vent was established.

The outage schedule provided the following sequence including appropriate logic ties to ensure sufficient hot leg vent was maintained at all times:

Various reasons may have Influenced the decision making process. The reasons may range from schedule familiarity to lack of procedure guidance. OP-4F, Reactor Coolant System Reduced Inventory Requirements, does not provide guidance on requiring the hot leg vent based on nozzle dam installation. The one line critical path schedule does not include all of the predecessors for any activity.

Immediate Action Taken: 4/9/2004 16:13:32 - HOLZMANN, MIKE:

Stopped installation of nozzle dams and had them removed from the S/G bowl until the pressurizer manway was removed. Once pressurizer manway removed, nozzle dams were installed and Unit 1 exited reduced inventory.

Place jobs in a safe condition and stop work.
