During performance of U1R28 NDE examinations required by NRC Order Issuance of Order Establishing Interim Inspection Requirements for Reactor Pressure Vessel Heads at Pressurized Water Reactors' possible flaw Indications were seen on Nozzle 26.

UT Examination PBNP 's UT examinations are being performed by Areva (formerly Framatome). The UT signal for nozzle 26 shows circumferential Indication In the Jgroove weld area that extends Into the CRDM tube base material.

The Areva UT analyst has concluded that the Indication Is fabrication-related and Is not a result of PWSCC cracking. The final documented Areva Inspection report has not yet been completed. Contact Brian Kemp for detailed Information.

PT Examination As a result of the above-mentioned UT examination, a dye-penetrant (PT) examination of the nozzle 26 CRDM J-groove material was performed. This was done to confirm the conclusion that no PWSCC cracking was occurring In nozzle 26.

The PT exams showed minor surface Indications that require further evaluation. There were two areas identified. The first if these areas are approximately 1 and Yi Inches in length by 5/8 Inches In width. The second area Is approximately 2 and *i Inches In length by 518 Inches In width. Each area Is comprised of several small linear Indications of various lengths. The final documented PT results are not yet Issued. Contact Bill Jensen for PT examination results.

Update on Nozzle 28 PBNP Engineering had directed that further examination of the J-grrove surface be performed following minor grinding of the weld surface. These exams have been completed. The results of the exams Indicate that flaws In the weld exist that do not meet the flaw evaluation guidance contained In WCAP-14000 'Structural Integrity Evaluation of Reactor Vessel Upper Head Penetrations to Support Continued Operation: Point Beach Units 1 &

2. Additionally, the exiatance of flaws In the weld region do not meet the understood acceptance criteria of NRC Inspection Order EA-03-09.

PBNP-11 Nozzle 26 will be repaired prior to placing the RPV Head back Into service.
