Dear Sirs:

As required by the PVNGS Technical Requirements Manual (TRM), Arizona Public Service Company (APS) is submitting this supplemental report to the PVNGS Unit 2, Cycle 12 Startup Report (Reference 1). TRM T5.0.600.2.c requires that supplemental reports be submitted at least every 3 months until all testing has been completed. At the time that Unit 2, Cycle 12 Startup Report (Reference 1) was initially submitted, two tests had not yet been performed. Those two tests were 1) steam generator moisture A member of the STARS (Strategic Teaming and Resource Sharing) Alliance Callaway 0 Comanche Peak U Diablo Canyon 0 Palo Verde 0 South Texas Project 0 Wolf Creek

U. S. Nuclear Regulatory Commission ATTN: Document Control Desk Unit 2, Cycle 12 Startup Report-Supplement 4 Page 2 carryover and, 2) steam generator blowdown piping vibration measurement. As reported in Reference 2, the remaining portions of the Steady State Vibration Testing for Unit 2 were completed on May 20, 2004.

Unit 2 performed Steam Generator Moisture Carryover Test on October 13, 2004. With Unit 2 at 100% power and steam generator blowdown isolated, a small amount of lithium hydroxide tracer was injected into the condensate header and allowed to come into chemical equilibrium in the steam generators. Samples were taken from various points in the secondary plant and shipped to an offsite laboratory for analysis. The average moisture carryover was determined to be 0.007% for Steam Generator #1 and 0.002% for Steam Generator #2. These results met the Acceptance Criterion that the average moisture carryover from the steam generators shall be < 0.1 %.

This letter completes all required reporting associated with Unit 2, Cycle 12 Startup testing per TRM T5.0.600.2.c.
