- 1.
There were no temporary suspensions or other administrative actions taken against individuals based upon on-site presumptive positive tests for marijuana or cocaine.
- 2.
One (1) contract worker terminated employment following a positive pre-access chemical test and was unavailable to meet with the MRO. Site access was terminated.
Eight (8) contract workers were denied unescorted protected area access following a pre-access chemical test failure. The individuals were provided detailed information regarding the right to appeal management actions resulting in access denial.
One (1) employee and one (1) contract worker were denied unescorted protected area access following a random chemical test failure. The employee was subject to actions called out in #7 below, and the contract worker was provided detailed information regarding the right to appeal management actions resulting in access denial.
- 3.
There were no transfers of licensee employees to non-nuclear positions (away from San Onofre site) as a result of failed chemical tests.
- 4.
The MRO reviewed one (1) employee's and nine (9) contract worker's appeal test results. The original test results were confirmed.
- 5.
There were no identified deficiencies in the Fitness for Duty program.
6 Four (4) contract workers had tests that were declared subversion attempts and/or refusal to test by the MRO. All were permanently denied site access as a result. The individuals were provided detailed information regarding the right to appeal management actions resulting in access denial.
- Two (2) contract workers submitted a high-temperature specimen for a pre-access test. Their oral temperature was within normal limits. They subsequently refused to provide a specimen under direct observation within the required timeframe. In each case, the MRO declared a test subversion attempt and refusal to test.
- One (1) contract worker submitted two (2) low-temperature specimens for a pre-access test. He subsequently provided a sample with an acceptable temperature that tested positive for opiates. The MRO declared a refusal to test based on the two cold samples that tested negative.

- One (1) contract worker tested positive for THC on a pre-access test. The individual admitted to providing false and misleading prescription medication information to the MRO who then declared a refusal to test.
- 7. One telephone notification was made to the NRC in accordance with 10 CFR 26.73 for this reporting period (NRC Operations Log #41 150). The notification text is as follows:
"October 26, 2004, Southern California Edison's (SCE) Medical Review Officer (MRO) met with an SCE employee (a supervisor) and at 1356 PDT confirmed positive results from a random fitness for duty test. The employee's Protected Area access was immediately suspended. SCE is submitting this report in accordance with 1 OCFR26.73(a)(2).
'The employee reported for a random FFD test on October 14, 2004 and initial presumptive testing was positive for illegal substances. The sample was sent off site for confirmation testing and confirmed the presumptive results. The MRO reviewed the test results with the employee on October 26, 2004.
'At the time of this report, Unit 2 was operating at approximately 100 percent power and Unit 3 was shutdown in a refueling outage. The NRC Resident Inspectors have been notified of this occurrence and will be provided with a copy of this report."
