- LOSS OF OFFSITE POWER / Page 1 of 52 LOSS OF FORCED CIRCULATION This procedure provides the operator actions which should be accomplished in the event of either a Loss of Offsite Power (LOOP) or a Loss .of Forced Circulation (LOFC). The actions in this procedure are necessary to ensure that the plant is placed in a safe, stable condition. The goals of this procedure are to mitigate the effects of a LOOP or LOFC, maintain the plant in hot standby, or hot shutdown or to establish Shutdown Cooling System entry conditions while minimizing radiological releases to the environment and maintaining adequate core cooling.

REV DESCRIPTION Page 10 ACT 2687663 - Entry Conditions, Step 2. b.; deleted the "Two GTG's in Parallel" 2 entry condition from the list of power sources when restoring from a Blackout.

ACTs 2649145, & 2655764 - Replaced 'LOOP has occurred' with 'loss of NAN- 5 S01 and NAN-S02 has occurred' to ensure that appropriate action is taken to prevent overpressurization of the condenser.

ACT 2636833 - Added an second condition to step 62 a. to ensure that charging 36 pumps are not being used for reactivity control prior to stopping all charging pumps to maintain pressurizer level. Added a second condition to step 62 b. to perform a cooldown to maintain pressurizer level if charging pumps are being used for reactivity control.

1. The Standard Post Trip Actions have been performed.

or BOTH of the following conditions exist:

  • A LOOP or LOFC initiated from Mode 3 or Mode 4
  • LTOP is NOT in service and
2. ANY of the following conditions exist:
a. Plant conditions indicate that a Loss of Offsite Power or a Loss of Forced Circulation has occurred. ANY or all of the following may be present or have occurred:
  • Transformer alarms
  • Breaker alarms
  • DGs automatically started
  • RCP trouble alarms
  • Condenser vacuum alarm
  • Low RCS flow indications or
b. A station blackout has occurred, and at least one vital 4.16 kV AC bus has been restored from ONE of the following sources:
  • Affected Unit Diesel Generator
  • Other Unit Diesel Generator
  • Offsite Power or
c. Subcooled natural circulation is to be used for RCS heat removal although at least one non-vital 13.8 kV AC bus is energized.

1. The diagnosis of a LOOP / LOFC is NOT confirmed.

or

2. ANY of the LOOP / LOFC Safety Function Status Check acceptance criteria are NOT satisfied.

or

3. The LOOP / LOFC procedure has accomplished its purpose by satisfying ALL of the following:
  • All Safety Function Status Check acceptance criteria are being satisfied
  • Shutdown Cooling Entry conditions are satisfied or plant cooldown is NOT required and maintaining Mode 3 or Mode 4 conditions is desired
  • An appropriate procedure to implement has been provided and administratively approved

  • 1. Confirm the diagnosis of a LOOP/LOFC 1.1 RediagnosetheeventandQGOTOONE by performing the following: of the following:
a. Check that the Safety Function
  • Appropriate Optimal Recovery Status Check acceptance criteria Procedure are satisfied.
  • Functional Recovery Procedure
b. Direct Chemistry to PERFORM 74DP-9ZZ05, Abnormal Occurrence Checklist.
  • 2. Ensure the event is being classified.
3. Open the Placekeeper and enter the EOP Entry Time.
  • 4. IF a LOOP has occurred, THEN ensure loads have sequenced on at least one 4.16 kV AC vital bus.

-CAUTION Starting a Charging Pump without first isolating seal injection will likely cause seal damage due to rapid cooldown of the seals.

  • 5. IF a LOOP has occurred, THEN perform the following:
a. IF no charging pumps are running, THEN isolate seal injection.
b. IFseal injection is isolated, THEN isolate controlled bleedoff.
c. Reset the anti pump condition on c.1 Select a new always running the always running Charging Charging Pump.

Pump by placing the handswitch in the "STOP" position.

d. IF seal injection was isolated, THEN restore seal injection.
  • 6. IF a loss of NAN-SO1 and NAN-S02 has occurred, THEN actuate MSIS.
  • 7. Check that Tc is being maintained less 7.1 Ensure Tisbeing maintained less than than 570'F by using ANY of the 570 0F by operation of ANY of the following: following:

  • 8. IF steaming to atmosphere, THEN inform Radiation Protection and the RMS Technician.
  • 9. Ensure at least one Steam Generator has level being maintained within or being restored to 45 - 60% NR.
  • 10. Check that pressurizer level meets 10.1 Operate charging and letdown to BOTH of the following: restore pressurizer level to 10 - 65%.
  • 10 - 65%
  • 0 Trending to 33 - 53%

  • 11. IF VCT makeup is NOT available, THEN perform the following:
a. IF RWT level is below or approaching 73%,

AND the CRS desires to keep charging in service, THEN PERFORM ONE of the following:

  • Appendix 10, Charging Pump Alternate Suction to the RWT

/ Restoration

  • Appendix 11, Charging Pump Alternate Suction to the SFP /

Restoration

b. IF RWT level is above 73%,

THEN perform the following:

1) IF three charging pumps will be used, THEN stop the Boric Acid Makeup Pumps.
2) IF three charging pumps are will be used, AND a Fuel Pool Cleanup Pump is recirculating the RWT, THEN so RWT recirc by stopping the appropriate Fuel Pool Cleanup Pump.
3) Open CHN-HV-536, RWT Gravity Feed to Charging Pump Suction.
4) Close CHN-UV-501, Volume Control Tank Outlet.

  • 12. Maintain RCS pressure within the P/T limits by performing the following.

REFER TO Appendix 2, Figures:

a. Control the cooldown rate.
b. Control pressurizer heaters and main or auxiliary spray.
c. IF Main Spray is being used with fewer than four RCPs running, OR Auxiliary Spray is being used, THEN PERFORM Appendix 6, Spray Valve Actuation Data Sheet.
d. IF SI throttle criteria are met, THEN control charging, letdown, and HPSI flow.
  • 13. IF RCPs are NOT operating, 13.1 Ensure proper control of Steam THEN check natural circulation flow in Generator feeding and steaming.

at least one loop by ALL of the following:

  • Loop AT is less than 651F
  • Hot and cold leg temperatures are constant or lowering
  • RCS is 24 0 F or more subcooled using CET Subcooling
  • Less than a 300 F AT between Th RTDs and the maximum quadrant CET temperature

  • 14. IF ALL of the following conditions exist:
  • Power has been lost to ANY SIT Outlet Valve
  • The 125 VDC bus supplying the vent valve of the same train as the affected SIT outlet valve is not being powered by a battery charger
  • The vent valve of the opposite train as the affected SIT outlet valve is known to be failed closed and can not be opened
  • Power will not be restored to either the outlet valve or battery charger within two hours of the loss of the battery charger THEN vent the affected SIT within two hours of the loss of the battery charger.

  • 17. Unit 1 only -

IF power is lost to WRF, AND power restoration is NOT expected within one hour, THEN contact WRF and direct the GTG Z operator to perform the following:

a. Start a GTG.
b. Energize NAN-S07.

CAUTION A lightning strike on the overhead lines between the switchyard and NAN-S03 may damage ESF Transformer NBN-X03 which will prevent energizing either vital 4.16 kV AC bus from the GTGs.

  • 18. Unit 1 only -

IF the switchyard is de-energized, AND BOTH of the following conditions exist:

  • Power restoration is NOT expected within two hours
  • Thunderstorm activity is NOT present in the vicinity of PVNGS THEN PERFQBM Appendix 54, Energize Switchyard Loads From the

The GTGs can not supply Units 2 and 3 simultaneously but can supply Units 1 and 2 or Units 1 and 3.

  • 19. IF one vital 4.16 kV AC bus is de-energized, AND restoration is desired, THEN PERFORM ANY of the following:
  • Appendix 55, Restore DG A to PBA-S03
  • Appendix 56, Restore DG B to PBB-SQ4
  • Appendix 57, Align the GTG to Vital Buses
  • Appendix 58, Cross-Tie DG B to PBA-S03
  • Appendix 59, Cross-Tie DG A-to PBB-S04

  • 20. IF a vital 4.16 kV AC bus is energized from the GTG, AND a Diesel Generator can be restored to the bus, THEN PERFORM ONE of the following:
  • Appendix 60, Align DG A to PBA-S03 (Energized from a GTG)
  • Appendix 61, Align DG B to PBB-S04 (Energized from a QTG)
  • 21. PERFORM Appendix 31, Local Monitoring of IA Pressure and Emergency Nitrogen Supply.
  • 22. IF nitrogen is being used as a backup to Instrument Air, THEN perform the following to minimize danger to personnel:
a. Start all available trains of Auxiliary Building Essential AFUs.
b. Announce the following over the site-wide PA system once an hour or more:

"Attention all personnel. An oxygen deficient atmosphere may exist in Unit # in valve galleries and in rooms below the 100 ft elevation. Oxygen monitors shall be used in or near these rooms.'
