b Dominion October 4, 2004 United States Nuclear Regulatory Commission Serial No.04-608 Attention: Document Control Desk NL&OS/ETS RO Washington, D.C. 20555 Docket No. 50-338 License No. NPF-4 VIRGINIA ELECTRIC AND POWER COMPANY NORTH ANNA POWER STATION UNIT 1 STEAM GENERATOR TUBE PLUGGING REPORT Scheduled eddy current inspections of the North Anna Unit 1 C steam generator were completed during the Fall 2004 Refueling Outage. In accordance with North Anna Power Station Technical Specifications, Reporting Requirement 5.6.7.a, this letter provides notification of a steam generator tube that was plugged during the outage.
The tube was damaged by a foreign object and had an indication of damage that exceeds the North Anna Technical Specification plugging limit of 40 percent through wall. The indication was found by Bobbin Probe Examination in tube SGC R37 C79. A loose part was also noted at the time of inspection and subsequently removed. The tube was then tested with a rotating pancake coil for sizing. Using three different approved sizing techniques resulted in the following assessments with the most conservative being 43 percent through wall.
