Dear Mr. Stinson:
This letter refers to the Nuclear Regulatory Commissions (NRC) Office of Investigations (OI) completed investigation regarding falsification of fire watch patrol logs at the Farley Nuclear Plant. The information gathered by OI during the investigation substantiated that a Nuclear Helper employed at the Farley Plant deliberately falsified fire watch records on February 28, 2004.
Based on the results of the investigation, a violation involving the failure to follow Administrative Procedure FNP-0-AP-39, Fire Patrols and Watches, and the requirements of 10 CFR 50.9, Completeness and Accuracy of Information, has been identified. The violation occurred on February 28, 2004, when a Nuclear Helper failed to conduct a fire watch for the emergency diesel generator tunnel, yet completed the patrol log to make it appear that he had.
For administrative purposes, this non-compliance will be documented as a licensee identified non-cited violation in an upcoming integrated inspection report. If you contest this violation, you should provide a response within 30 days of the date of the upcoming inspection report, as described therein.
