CR-VTY-2003-02559; SAH-VG-5A1 FAILED THE ALARM RESPONSE TEST ON THE OXYGEN MONITORING DRAWER.

DATE: DECEMBER 10, 2003 PRO NUMBER: PRO-032559 EVENT DESCRIPTION:

On 11/24/03 with the reactor at full power, the NA! H202 Monitor SAH-VG-5A1 failed the alarm response test on the Oxygen monitoring drawer that resulted Inthe recorder failing to respond to oxygen Indication. The OA' H202 Monitor was declared inoperable per TS 3.2.6 The following regulations were considered when determining reportability of this event Operation or Condition Prohibited by Technical Specificatlons

§50.73(a)(2)(B) "(The licensee shall report:I Any operation or condition which was prohibited by the plantes Technical Specifications except when:

NUREG 1022, Rev. 2, States the following in section 3.2.2(1): "For testing that is conducted within the required time (Le., the surveillance interval plus and allowed extension), it should be assumed that the discrepancy occurred at the time of its discovery unless there is firm evidence, based on a review of relevant information such as the equipment history and the cause of the failure, to indicate that the discrepancy existed previously."

Event or Condition That Could Have Prevented the Fulfillment of a Safety Function

§50.73(a)(2)(v) "[The licensee shall report:] Any event or condition that could have prevented the fulfillment of the safety function of structures or systems that are needed to:

(D) Mitigate the consequences of an accident" 1

^ - -1 DISCUSSIONIBASES:

The function of the CAD system with regard to the sample sub system is to provide necessary Indications for containment H2 /02 levels. This Includes Monitors and Recorders. The Monitors and Recorders were tested just prior to this event and were found to be working correctly. This portion of OP 4344 inserts a dummy signal using a test switch to test alarm functions. The alarm functions and monitors did not respond as expected. This Is a failure of the test switch to Insert the proper signal. No functions were lost due to this test switch failure.

On 11/24/03 at 1240, the OA" H202 monitor (SAH-VG-5A1) was declared inoperable due to failure of the alarm function. As discussed above this was not a failure of the alarm function, but a failure of the test switch to Insert the correct signal to test the alarm. On 11125/03 at 1230, the "A H202 monitor was declared operable following satisfactory l&C functional testing.

SUMMARY

The safety function of this system was not affected or compromised as a result of the failure of the test switch. The appropriate LCO was entered pending repair and a Maintenance Rule Functional Failure deternination.

CONCLUSION: This event Is not reportable pursuant to §50.73(a)(2)(B) or §50.73(a)(2)(v)(D).
