NOTIFICATION OF FARLEY NUCLEAR PLANT - REVIEW OF GENERIC LETTER 89-13: SERVICE WATER SYSTEM PROBLEMS AFFECTING SAFETY-RELATED EQUIPMENT INSPECTION
Dear Mr. Stinson:
The purpose of this letter is to notify you that the U.S. Nuclear Regulatory Commission (NRC)
Region II staff will conduct a review of Generic Letter 89-13, Service Water System Problems Affecting Safety-Related Equipment, at your Farley Nuclear Plant during the week of September 7, 2004. A group of four inspectors will perform this inspection. The inspection will be led by Mr. Michael A. Scott, a Senior Reactor Inspector from the NRC Region II Office. This inspection will be conducted in accordance with a Temporary Instruction (TI) Review of Generic Letter 89-13: Service Water System Problems Affecting Safety-Related Equipment.
The objective of this TI is to support the NRC review of licensees activities in response to NRC generic communications. This TI will gather information to help the NRC staff identify and shape possible future regulatory positions and enhance the agency operating experience program. The inspection will verify actions implemented in response to NRC Generic Letter 89-13 are programmatically controlled and have been maintained effective.
During a telephone conversation on July 08, 2004, Mr. Jack Kale of your staff and myself, confirmed arrangements for the week onsite inspection (September 7-10, 2004) and the information request enclosed. The purpose of the information request is to obtain information and documentation, outlined in the Enclosure, that is needed to support the inspection. Please contact Mr. Scott prior to preparing copies of the materials listed in the Enclosure. The inspectors will try to minimize your administrative burden by specifically identifying only those documents required for inspection preparation. Please provide the information requested by August 18, 2004.
In accordance with 10 CFR 2.390 of the NRC's "Rules of Practice," a copy of this letter and its enclosure will be available electronically for public inspection in the NRC Public Document Room or from the Publicly Available Records (PARS) component of NRC's document system (ADAMS). Enclosure INFORMATION REQUEST FOR TEMPORARY INSTRUCTION, REVIEW OF GENERIC LETTER 89-13: SERVICE WATER SYSTEM PROBLEMS AFFECTING SAFETY-RELATED EQUIPMENT INSPECTION
- Please provide the information electronically in searchable.pdf files on CDROM if possible.
The CDROM should be indexed and hyperlinked to facilitate ease of use. Information in lists should contain enough information to be easily understood by someone who has a knowledge of pressurized water reactor technology. Hard copies are an acceptable alternative for those items which are difficult to place on CDROM.
- Time frame for data will be three years forward. Selected heat exchangers are all safety-related heat exchangers (HXs) cooled by the Service Water (SW) system.
1.
Piping and instrumentation drawings for SW system (Paper copies (2) are preferred for these).
2.
Plant Technical Specifications, Bases, and Technical Requirements Manual sections involving SW.
3.
A current copy of the Updated Final Safety Analysis Report including the living FSAR sections affecting SW.
4.
System description/design basis document for SW.
5.
Operator training modules for SW.
6.
Response(s) for Generic Letter 89-13 and any commitment changes since.
7.
Design information associated with all safety-related (SR) HXs fed by SW.
8.
Tube plugging information for all SR HXs fed by SW, as applicable.
9.
List of testing/inspections performed to assure ultimate heat sink performance, such as SW intake inspections, special erosion/corrosion inspections, flow balance tests, piping inspections, fouling inspections, pond/dam inspections, etc. and a copy of the associated procedures for these tests/inspections.
10.
List of surveillances and a copy of associated procedures used to ensure the operability of SW equipment and test result data.
11.
Heat exchanger (HX) performance data such as heat transfer tests, chemistry data, PM information, as applicable for all SR HXs fed by SW.
12.
Trending data for these SR HXs.
13.
The normal operating procedures for the SW system. Include the procedures which specify the normal valve alignment for this system.
14.
Emergency Operating Procedures (EOPs), Abnormal Operating Procedures (AOPs),
and Alarm Response Procedures involving SW. Note: Please make available, onsite during the inspection, the supporting documents such as the EOP basis documents, step deviation document, writers guide, users guide, and calculations used to support the setpoints in EOPs.
15.
A list of engineering calculations applicable to SW.
16.
A list of temporary modifications and operator work-arounds involving SW.
17.
A list of operability determinations involving SW.
18.
Lists of corrective action program documents, LERs, findings, and non-routine work requests initiated for SW system and components.
19.
System Health Reports and system performance trends for SW.

A copy of the Maintenance Rule program procedure and performance criteria for SW and a list of Maintenance Rule functional failures for SW.
21.
Quality Assurance audits, self-assessments and third party assessments performed on SW.
22.
A list of Operating Experience Program evaluations of industry, vendor, or NRC generic issues.
23.
A list of all SW permanent plant changes, design changes, setpoint changes, major procedure changes, equivalency evaluations, suitability analyses, and commercial grade dedications.
24.
A copy of plant procedures that provide the guidelines for the following programs: 1)
Corrective Action Program, 2) Program for the evaluation of changes, tests, and experiments [50.59 Program] 3) Program for the incorporation of local and industry operating experience into Farley programs and procedures.
