Dear Sirs:

In accordance with Technical Requirements Manual (TRM) requirement T5.0.600.2.c, Arizona Public Service Company (APS) is submitting a supplement report to PVNGS Unit 2, Cycle 12 Startup Report (Reference 1). TRM T5.0.600.2.c requires that supplemental reports be submitted at least every 3 months until all testing has been completed. At the time that Unit 2, Cycle 12 Startup Report (Reference 1) was submitted, two tests had not yet been performed. Those two tests were 1) steam generator moisture carryover and, 2) steam generator blowdown piping vibration measurement. On May 20, 2004 with Unit 2 at approximately 99% power, the remaining portions of Steam Generator Replacement Project (SGRP), Steady State Vibration Test were completed. Flow was established for normal, abnormal and high rate alignments and vibration levels were measured on several steam generator blowdown valves. The highest vibration reading was 0.541 inches per second on the valve actuator for SCNHV1 8B (S/G 1 Abnormal Rate to the Condenser). All of the points measured were well below the Acceptance Criterion of 2.5 inches per second.

Steam Generator Moisture Carryover Testing has not yet been completed. A supplement report will be provided when this test is completed.
