TERRANCE REIS NRR 21.21 UNSPECIFIED PARAGRAPH PART 21 NOTIFICATION INVOLVING VOLTAGE MONITORING RELAYS "The following information is provided pursuant to the requirements of 10CFR Part 21 to report a potential safety concern. A potential safety condition may exist with the Eaton Cutler-Hammer D65 Series BI Voltage Monitoring Relay that can cause the unit to bypass the Manual Reset feature. This could result in the D65 relay being energized without an operator manually resetting it resulting in possible personal injury or death or property damage. This condition exists with Eaton Cutler-Hammer D65 Series BI Voltage Monitoring Relays with a manufacturing date between May 12, 2002 and February 20, 2004.
"On March 25, 2004, Framatome/AREVA Electrical Products received the attached Eaton Cutler Hammer Product Safety Warning for D65 Voltage Monitoring Relays, dated March 24, 2004. The Product Safety Warning states that Eaton's Cutler-Hammer D65 Series BI Voltage Monitoring Relays with a manufacturing date identified above may power up without being manually reset even when a NC [Normally closed] switch is wired to activate manual reset mode. This condition is only present when used in applications operating in the manual reset mode. This condition is not present when the device is used in automatic reset mode. This condition may only result when the D65 Series BI is used and will not result if the Al Series is used (as the Al series did not offer the potential for manual reset).
"The supply of Safety Related D65 Voltage Monitoring Relays by Framatome/AREVA Electrical Products is isolated to only one Nuclear Customer (i.e. First Energy - Davis Besse Nuclear Power Plant). The First Energy Davis Besse Nuclear Power Plant was notified by Framatome/AREVA Electrical Products on April 2, 2004."

This condition Is only present when used In applications operating In the manual reset mode. This condition is not present when the device is used in automatic reset mode. This condition may only result when the D55 Series Si Is used and will not result I the Al Series Is used (as the Al series did not offer the potential for manual reset).
The logic board of the relay is being powered by two of the three phases. It is Phases B and C for the surface mount units (style numbers beginning with D65VMLS) and Phases A and C for the plug-in units (style numbers beginning with D6SVMLP). If either of the phases being used to power the logic board Is lost, the logic board configurations are lost and the unit will go into Power Up mode upon clearing of the fault The unit will not operate as If It is wired for Manual Reset Mode.

The date code of the unit can be determined by finding the four-digit numerical code on th6 lower left side of the product label located on the left side of the unit. The right side of the unit has a label to identify the type of fault which has occurred based upon LED status. The date code is on the label on the opposite side of the unit The first two digits of the date code represent the year of manufacturer while the second two digits represent the month of manufacture.
If a D65 Series BI Voltage Monitoring Relays with a Manufacturing Date Code between 0320 and 0408 Is being utilized and the application requires manual reset, replacement of the unit Is required. If the automatic reset mode is being utilized, the unit will operate properly and Is not subject to the condition identified in the attached Eaton Cutler-Hammer Product Safety Warning, however, replacement of the unit is suggested by Eaton Cutler-Hammer. If the unit Is not replaced for any reason, the units must be labeled to explain to any future user not to use the unit In the manual reset mode.

This could result In the D65 relay being energized without an operator manually resetting It resulting In possible personal injury or death or property damage. This document describes which products and what applications are affected, and the corrective actions required.
If you are using a D65 relay with the Manual Reset feature, the unit must be replaced. As part of our standard warranty process, Eaton will arrange to send you replacement devices In advance to facilitate easier replacement The replacement D65 products are Identified as Series 82 devices.
A modification has been made to the Software for this device that will permanently eliminate any potential for this condition occurring In the future. When In the manual reset mode, the reset button must be opened on power up and after a fault has been cleared to allow the unit to operate normally.
In addition, the adjustable time delay (1-300 seconds) now applies both on power up and on restart after a fault has been cleared. All other features of the product remain the same-Description Eaton's Cutler-Hammer D65 Series B1 Voltage Monitoring Relays with a Manufacturing Date between May 12, 2003 and Februay20,2004 may power up without being manually reset-even when a NC switch Is wired to activate manual reset mode. This could result In personal Injury, 'equipment damage, or death. This condition Is only present when used in applications operating In the.
manual reset mode. This condition is not present when the deVice Is used in automatlC ret mode. If you are using automatic reset mode, the unit will operate properly and Is not subject to the condition described In this document. However, we suggest that you return the unit for a replacement device through standard warranty procedures. If you decide not to replace the unit for any reason, you must apply the enclosed sUcker somewhere visible on the unit to explain to any future user not to use the unit In the manual reset mode.
