SAN ONOFRE NUCLEAR GENERATING STATION - CONFERENCE CALL
SUMMARY
REGARDING REGENERATIVE HEAT EXCHANGER WELD FAILURE On February 9, 2004, the staff held a conference call with Southern California Edison (SCE) regarding PNO-IV-04-003, "Southern California Edison, Unit 3 Shutdown to Repair a Letdown System Piping Leak." A summary of the event is available in ADAMS under accession number ML040270293. The list of participants in the conference call is attached.
During the call, SCE presented the initial root cause findings and their determination of the failure mechanism to the staff. SCE attributed the cracked weld at San Onofre Nuclear Generating Station, Unit 3 to excessive vibration from adjacent piping and pump operating combinations. The staff asked additional questions pertaining to the licensees examinations and methodology. The staff also requested to review the licensees nondestructive examination data and final metallurgical evaluation reports. The licensee agreed to supply these upon the completion of their evaluation.
