Dear Mr. Wood:

In response to your facility letters dated October 4, 1999, and May 10, 2000, we have tentatively scheduled initial licensing examinations for your operator license applicants at the Perry Nuclear Power Plant during the weeks of March 4 and March 11, 2002, and January 6 and January 13, 2003. Validation of the examinations will occur at the station during the weeks of February 18, 2002, and December 16, 2002, respectively. In the unlikely event that we are unable to support the examination during the scheduled weeks, we will inform you immediately upon discovery of such conditions and make arrangements to administer the examination at a mutually acceptable date.

As agreed upon in a telephone conversation between R. Strohl, Perry, and A. M. Stone, NRC, on January 3, 2001, your staff will develop the examinations. To support the examination administration dates, we have tentatively scheduled the dates of January 14, 2002, and December 2, 2002, respectively, to begin our review of your submitted examinations.

Your staff indicated you are training approximately 12 candidates for each examination. Please inform us if the number of candidates declines below 10 as this will impact the examination schedule. Please also inform us at your earliest opportunity if you discover you are unable to support the examination on the scheduled dates.

Once your staff has determined a schedule for examination development, please have them contact the Chief Examiner to arrange for a suitable examination outline submittal date for NRC review. The intent is for the examination outline to be submitted early in the examination development process. This is to preclude the need to make significant changes to developed examination material as a result of the NRC review of the outline. Mr. Jay A. Hopkins, has been tentatively assigned as the Chief Examiner for the March 2002 examinations and can be reached at 630-829-9739. Mr. Michael E. Bielby, Sr. has been tentatively assigned as the Chief Examiner for the January 2003 examinations and can be reached at 630-829-9762.

A supplementary letter will be sent to the training department approximately 120 days prior to the examination outlining examination security expectations, listing the materials required by the NRC to conduct the examination, reconfirming the examination dates, and reconfirming the number of candidates you have in the training program. Dear Mr. Kanda:

In a telephone conversation on September 9, 2002, between Mr. J. D. Pierson, Exam Lead, and Mr. Mike Bielby, Chief Examiner, arrangements were made for the administration of licensing examinations at the Perry Nuclear Power Plant the weeks of January 27, 2003, and February 3, 2003. In addition, the NRC will make an examination validation visit to your facility the week of January 6, 2003.

As agreed during the telephone conversation, your staff will prepare the examinations based on the guidelines in Revision 8, Supplement 1, of NUREG-1021, "Operator Licensing Examination Standards for Power Reactors." The NRC regional office will discuss with your staff any changes that might be necessary before the examinations are administered.

To meet the above schedule, it will be necessary for your staff to furnish the examination outlines by October 21, 2002. The written examinations, operating tests, and the supporting reference materials identified in Attachment 2 of ES-201 will be due by December 2, 2002.

Pursuant to 10 CFR 55.40(b)(3), an authorized representative of the facility licensee shall approve the outlines, examinations, and tests before they are submitted to the NRC for review and approval. All materials shall be complete and ready to use. Any delay in receiving the required examination and reference materials, or the submittal of inadequate or incomplete materials, may cause the examinations to be rescheduled.

In order to conduct the requested written examinations and operating tests, it will be necessary for your staff to provide adequate space and accommodations in accordance with ES-402, and to make the simulation facility available on the dates noted above. In accordance with ES-302, your staff should retain the original simulator performance data (e.g., system pressures, temperatures, and levels) generated during the dynamic operating tests until the examination results are final.

Appendix E of NUREG-1 021 contains a number of NRC policies and guidelines that will be in effect while the written examinations and operating tests are being administered.

To permit timely NRC review and evaluation, your staff should submit preliminary reactor operator and senior reactor operator license applications (Office of Management and Budget (OMB) approval number 3150-0090), medical certifications (OMB approval number 3150-0024),

and waiver requests (if any) (OMB approval number 3150-0090) at least 30 days before the first examination date. If the applications are not received at least 30 days before the examination date, a postponement may be necessary. Signed applications certifying that all training has been completed should be submitted at least 14 days before the first examination date.

In accordance with NUREG-1021, ES-201, enclosed are the proposed examinations, supporting documentation, and reference materials for the NRC initial license examination tentatively scheduled to be administered starting January 27, 2003. The examination materials were developed in accordance with the guidelines specified in NUREG-1 021, Revision 8, Supplement 1.

NUREG-1021 Forms ES-301-3, ES-301-4, ES-301-5, ES-301-6, ES-401-7, and ES-D-2 are enclosed with the supporting documentation. Written exam questions are on facility generated sheets with the same information as contained on Form ES-401-6.

Form ES-301-6 has been completed based on the projected class size and license type at this time. A listing of proposed simulator scenario crew assignments is also included which supports Form ES-301-6. The class size and makeup may change prior to the examination date.

The NRC initial license examination outlines were previously submitted with a cover letter dated August 8, 2002. Subsequent changes to the written examination outlines were requested by the facility author and have been approved since the outlines were submitted.

These changes are documented on Form ES-401-10 for each examination outline. A complete set of revised outline forms for both the RO and SRO examinations are included in this submittal.

The Operating Test outlines for the applicant's audit exams have also been included in this submittal as requested.

It is hereby requested that the enclosed examination materials be withheld from public disclosure until after the initial license examinations are completed.

In accordance with NUREG-1 021, ES-501, Section C. 1, the following items are submitted following completion of the Perry written examinations and operating tests during the period of January 27, 2003 through February 6, 2003.

1. The results of the written examination performance analysis.

Copy of Forms ES-201-3, "Examination Security Agreement," with a pre examination and post-examination signature by every individual, except one, who had detailed knowledge of any part of the written examination or operating tests before they were administered.

One Perry Training Section instructor has been called out of town on family business and is unavailable to sign the post-examination signature at this time. In a telephone conversation with you on February 10, 2003, you stated that this would not delay the issuance of licenses. On your suggestion, we agreed to send you a copy of Form ES-201-3 at this time, with the originals to follow after this individual signs the post examination signature upon his return to the area.

No changes were made to the master examinations and answer keys during the administration and grading of the examinations. Therefore, the master examinations and answer keys will not be forwarded.

In our telephone conversation on February 10, 2003, you stated that Form ES-403-1, "Written Examination Grading Quality Assurance Checklist," would be filled out by you, therefore, this form will not be forwarded.

The applicants made no substantive comments after the written examination.

Therefore, no changes to the exam or answer keys will be requested.

Attachment-NRC Initial License Examination Analysis In accordance with ES-403, an exam analysis was performed in order to evaluate any applicant's knowledge weakness or training program deficiency. An evaluation was performed for questions that were missed by 50% or more of the applicants. When a question was evaluated to have been missed due to a knowledge weakness, it was determined that the training program contained sufficient coverage of the topic as to not warrant any further changes. The results of this evaluation are summarized below.

I.

RO/SRO #8 - Six of twelve applicants missed the question. The question was evaluated as valid with no changes required. Applicants missed the question due to a knowledge weakness regarding automatic isolation signals.

2.

RO/SRO #16 - Six of twelve applicants missed the question. The question was evaluated as valid with no changes required. Applicants missed the question due to a knowledge weakness regarding power supplies and the impact of a loss of power on NSSSS.

3.

RO/SRO #44 - Six of twelve applicants missed the question. The question was evaluated as valid with no changes required. Applicants missed the question due to a training program deficiency. Action will be taken to add instruction on this circuit to the lesson plan.

RO/SRO #45 - Ten of twelve applicants missed the question. The question was evaluated as valid with no changes required. Applicants missed the question due to a knowledge weakness regarding SRM/IRM detector operation and differences in detector construction.

5.

RO/SRO #61 - Eight of twelve applicants missed the question. The question was evaluated as valid with no changes required. Applicants missed the question due to a knowledge weakness regarding the effect of a loss of DC control power on the Reactor Recirculation Pump circuit breakers.

6.

RO/SRO #65 - Six of twelve applicants missed the question. The question was evaluated as valid with no changes required. Applicants missed the question due to a knowledge weakness regarding the inter-relations between opening the MSIVs and swell in the RPV.

In addition, a procedure enhancement will be requested in order to identify the plant conditions under which this may occur.

RO # 95 - Three of five applicants missed the question. The question was evaluated as valid with no changes required. Applicants missed the question due to a knowledge weakness regarding system inter-relations and instrumentation.

8.

SRO #85 - Four of seven applicants missed the question. The question was evaluated as valid with no changes required. Applicants missed the question due to a knowledge weakness regarding Technical Specification Bases.

9.

SRO # 87 - Four of seven applicants missed the question. The question was evaluated as valid with no changes required. Applicants missed the question due to a knowledge weakness regarding the proper names of communication systems.
