ENCLOSURE 3 TO AEP:NRC:3071 DONALD C. COOK NUCLEAR PLANT COMMITMENT CHANGE

SUMMARY

REPORT to AEP:NRC:3071 Page 2 CC-0196 Independent verification of equipment removal and return to service.

Affected Unit(s): Both Units Commitment No: 7533 Classification: Revision Source Document: AEP:NRC:0265F, dated February 23, 1984, Update of the Regulatory Performance Improvement Program.

Original Commitment Statement:

Effective February 1, 1984, independent verification of the removal and return to service of equipment shall be performed by persons acting independently of those performing the removal or return to service action. This action will be addressed in PMP-4043-ICV-001.

Change Description Summary:

A provision to clarify the use of Concurrent Verification for high voltage breaker alignments was added.

Revised Commitment Statement:

The task of Independent Verification for the removal and return to service of equipment shall be performed by persons acting independently of those performing the removal or return to service action. The applicability and limited use of Concurrent Verification is acceptable for electrical breaker restoration activities with voltages greater than 480 VAC and 250 VDC. Guidance for these actions are specifically addressed in PMP-4043-ICV-001, Independent and Concurrent Verification.

The procedure for use of the air- line hood shall be written by October 1, 1978, to assure the following:

(1) Only a manifold with a pressure gauge calibrated within the previous 12 months may be used. When in operation the pressure shall be kept at 70 ? 10 psi.

(2) A protection factor of 1000 shall be applied unless condition 3 below applies.

(3) If the hood is operated with the belt mounted regulating valve set at its full-open position, a protection factor of 2000 may be used.

Change Description Summary:

Item 1 was revised to refer to following manufacturers guidance. Items 2 and 3 were deleted, as the protection factors are included in 10 CFR 20 Appendix A.

Revised Commitment Statement:

When utilizing supplied air hoods, only a manifold with a pressure gauge calibrated within the previous 12 months may be used. When in operation, the pressure shall be maintained in accordance with manufacturers guidance for the equipment being used.

Original Commitment Statement:

Develop IST training lesson plans and modules before September 30, 2001.

Change Description Summary:

The commitment is being changed to reflect the new negotiated due date of November 30, 2001, to incorporate the performance of the training.

Revised Commitment Statement:

Develop IST training lesson plans and modules, and complete training before November 30, 2001 to AEP:NRC:3071 Page 5 CC-0209 Two valves not to be removed from environmentally qualified equipment list.

Affected Unit(s): Both Units Commitment No: 2165 Classification: Revision Source Document: AEP:NRC:0775AK, dated January 21, 1987, Environmental Qualification (EQ) of Limitorque Valves.

Original Commitment Statement:

Two valves [2-WMO-724 and 2-WMO-726] will not be removed from the environmentally qualified equipment list. Their qualification will be maintained following existing procedures.

Change Description Summary:

Valve 2-WMO-724 will be removed from the environmentally qualified equipment list.

2-WMO-724 no longer performs any safety functions and is removed from the EQ list by 2-DCP-5153 Revised Commitment Statement:

Valve 2-WMO-726 will not be removed from the environmentally qualified equipment list. Its qualification will be maintained following existing procedures.

Original Commitment Statement:

Although not stated in the procedure, the licensee stated they plan to replace the accelerometers and charge converters in Unit 1 every refueling outage, and are evaluating whether to replace those components in Unit 2 at the same frequency or during every other outage, as is the current practice.

Change Description Summary:

The 18 month Technical Specification surveillance procedure 12-EHP-4030-STP-234 has been superceded by 12-IHP-4030-002-002 Revision 0 Acoustic Valve Monitoring System Channel Calibration.

The procedure now provides directions for determining the date that the accelerometers and charge converters were last replaced. The procedure also provides directions based on Environmental Qualification (EQ) specification DCC-QA-105-QCN, EQ Equipment List, to determine whether they must be replaced during performance of the test procedure to assure that the qualified life of the components is not exceeded.

Revised Commitment Statement:

The 18 month AVMS channel calibration procedure provides directions to determine the date that accelerometers and charge converters were last replaced, and, based on the Environmentally Qualified Equipment List, if they must be replaced during the current performance of the calibration procedure to assure that their qualified lives will not be exceeded.
