Nine Mile Point Unit 1 Docket No. 50-220 License No. DPR-63 Special Report, Channel #11 Containment High Range Radiation Monitor Declared Inoperable to Repair Failed Chart Recorder Gentlemen:

In accordance with Action Statement 3.a of Nine Mile Point Unit 1 Technical Specification Table 3.6.11-2, "Accident Monitoring Instrumentation Action Statements," Nine Mile Point Nuclear Station, LLC is submitting the following Special Report concerning the inoperability of Channel #11 Containment High Range Radiation Monitor.

Description of Event On October 17, 2002, at approximately 0820 hours0.00949 days <br />0.228 hours <br />0.00136 weeks <br />3.1201e-4 months <br />, operators declared the Channel #11 Containment High Range Radiation Monitor inoperable in order to troubleshoot and repair the Channel #11 Containment High Range Radiation Monitor chart recorder.

The redundant Channel #12 Containment High Range Radiation Monitor was operable while the Channel #11 Containment High Range Radiation Monitor was out of service.

Cause of Event The cause of the Channel #11 Containment High Range Radiation Monitor chart recorder failure was a failed servo motor amplifier board.

1. The Channel #11 Containment High Range Radiation Monitor chart recorder servo motor amplifier board was replaced and the chart recorder calibrated.
2. On October 17, 2002, at approximately 1120 hours0.013 days <br />0.311 hours <br />0.00185 weeks <br />4.2616e-4 months <br />, the Channel #11 Containment High Range Radiation Monitor was returned to service.
3. The Containment High Range Radiation Monitor chart recorder failure has been entered into the site's corrective action program to assess the need for additional follow up actions.
